Effects Of The Exercise Related Interventions During Hemodialysis On Physical Motility And Health Condition Of MHD Patients

1 ObjectiveThe purpose of the present research project is to explore the effects of exercise knowledge education or combined endurance and resistance exercise during hemodialysis on physical motility and health condition in maintenance hemodialysis(MHD)patients. The results are desired to be able to provide better guidance and reference for promoting MHD patientrehabilitation.2 MethodsThis project is a randomized controlled study. Subjects were the patients who accepted treatment in the Blood Dialysis Ward of The Suzhou City Hospital from March2015 to March 2016. Totally 75 subjects who meet the requirements were included and randomly divided into Control group, Education group and Exercise group with each group 25 patients. The Control group received the regular dialysis without special treatment. During regular dialysis, patients were allowed to watch the TV programs. Instead of watching TV programs, the Education group watched VCD and PPT of exercise knowledge for one hour each time and three time each week. The Exercise group received exercise instruction and performed endurance exercise and resistance training for one hour each time and three time each week. Both interventions lasted for 12 weeks. Before and after the intervention, patienthealth conditions and physical motility were examined, which included weight, body mass index, resting heart rate, blood pressure, body fat and muscle quantity, bone density, ventilatory anaerobic threshold,grippingstrength and 6 minutes walking distance test. Other exercise relatedparametersincluded patient’s willingness of exercise, their understanding of movement therapy, dialysis patient exercise benefits/barrier scale(DPEBBS), activities of daily living, etc. All data were analyzed using SPSS20.0 statistic software.3 ResultsThe examined parameters did not show significant difference in the Control group. After receiving 12-week exercise knowledge education, the Education grouppatients’ willingness of exercise was enhanced(p<0.05), their understanding of movement therapy was improved greatly(p<0.05), and scoring of DPEBBS was also improved markedly(p<0.05). The exercise group had the similar changes with the Education group on the above parameters after12-week combined endurance exercise and resistance training. In addition, the Exercise group had reduced resting heart rate, decreased systolic pressure(p<0.05), increased ventilatory anaerobic threshold(p<0.05), enhanced gripping strength and farther 6 minutes walking distance(p<0.05)after intervention.4 ConclusionsThe 12-week exercise knowledge education during dialysis for the MHD patients can improve their recognition and initiative of exercise and reduce their feeling of movement disorder. The 12-week combined endurance exercise and resistance training not only provides the similar benefits with education, but also improves the MHD patient physical motility and cardiovascular health.
