Objective:To investigate the therapeutic mechanism of Xinkaikujiang method in a rat model of Reflux esophagitis model rats (RE).Methods:98 wistar male rats were randomly divided into a control group (10) and a control group (88). All control group rats were induced by improved part cardia myotomy+partial external pyloric ligation.28 rats were killed after modeling. The control group of the remaining 60 rats were randomly divided into a model group, a Traditional Chinese Medicine (TCM) low-dose group, a TCM middle-dose group, a TCM high-dose group, a mixed group of Western medicine, a Western medicine group,10 in each. Not treated normal group. Model group was treated with 0.9% NS, while the TCM low-,middle-,high-dose groups were orally administered respectively with 0.06454 g/ml.0.12908 g/ml and 0.25816g/ml once each morning and evening. The western group was given Domperidone 0.19362 mg/ml, Hydrotalcid 0.009681 g /ml. Rabeprazole 0.12908 mg/ml of mixed liquid each morning and evening. The mixed group of Western medicine was given the mixed liquid of middle-dose group and western group each morning and evening. According to the different treatment time, each group was divided into 7 days,14 days two subgroups, each subgroup of 5 rats. We measured esophageal pH value, observed the esophagus specimens and esophageal mucosa histopathological changes. One another using immunohistochemistry and western-blotting method to detect the Carbon monoxide synthase (NOS2) of esophageal mucosa respectively in the treatment of 7 and 14 days.Result:1.The impact of Xinkaikujiang method on rats lower esophageal mucosa PH value:After seven days, lower esophageal PH value of model group is significantly lower than the normal group (P<0.01), while mixed group, TCM low-dose group and western medicine group compared with model group, the lower esophageal PH value were increased, but not statistically significant (P> 0.05). it was significantly higher in TCM middle-dose group compared with the model group in lower esophageal PH value (P<0.01). TCM high-dose group is lower than model group in PH value.but it was not statistically significant (P> 0.05). Afterl4 days. TCM high-dose group and mixed group, TCM middle-dose group and western medicine group compared with the model group in lower esophageal PH value have increased, there was significant difference (P<0.05), the lower esophageal PH value of TCM low-dose group is higher than model group (P> 0.05). The lower esophageal PH value of Mixed group is increased significantly compared with the TCM low-dose group (P<0.05), the lower esophageal PH value of TCM middle-dose group is lower than western medicine group dose (P>0.05).2.Effect Xinkaikujiang method of esophageal tissue morphology in RE rats: After 7 days, model group and the normal group there are significant differences in tissue morphology (P<0.05); model group compared with TCM high-dose, middle-dose group, low-dose group, mixed group and western group are not statistically significant (P> 0.05). After14 days treatment, model group compared with TCM low-dose, middle-dose group, high-dose group, mixed group and western group are statistically significant (P<0.05), and among TCM low-dose, middle-dose group, high-dose group, mixed group and western group there is no statistically difference(P> 0.05).3.Effect Xinkaikujiang method of rats lower esophageal pathological morphology:After 7 days treatment, TCM low-dose group, TCM middle-dose group, mixed group and western group showed infiltration of inflammatory cells in the epithelial cell layer, part squamous cell hyperplasia, lamina propria papillae extend to the grassroots, severe mucosal erosions; TCM high-dose group showed squamous cell hyperplasia, visible part of the infiltration of inflammatory cells in the lamina propria of connective tissue. After 14 days treatment, TCM low-dose group showed squamous cell is incomplete, a small amount of inflammatory cell infiltration; TCM middle-dose group showed squamous cells extending from the sides toward the middle to form a complete epithelial layer, ulcer repair, a small amount of inflammatory cell infiltration; TCM high-dose group showed squamous cell layer is not complete, epithelial cell layer infiltration of inflammatory cells; mixed group showed epithelial cell layer infiltration of inflammatory cells; western medicine group showed a small amount of inflammatory cell infiltration, squamous keratosis hyperplasia.4.After Xinkaikujiang method treatment of RE Rats, determination of the lower esophageal tissue by Immunohistochemistry of NOS2:After 7 days treatment, the model group compared with the normal group in NOS2 expression of rats lower esophageal was increased (P<0.05), each group comparing in rats lower esophageal NOS2 expression with model group have varying degrees of reduction (P<0.05). After 14 days treatment, TCM high-dose group, mixed group and western medicine group compared with model group in rats lower esophageal NOS2 expression have varying degrees of reduction (P<0.05), but TCM high-dose group, mixed group and western medicine group in NOS2 expression have not significant difference (P> 0.05), TCM low-dose group and middle-dose group compared with the model group have little difference (P> 0.05).5.After Xinkaikujiang method treatment of RE Rats, determination of the lower esophageal tissue by western-blotting of NOS2:After 7 days treatment, the model group compared with the normal group in NOS2 expression of rats lower esophageal was increased(P<0.01), each comparison with the model group of rats lower esophagealNOS2 expression was not statistically significant (P> 0.05). After 14 days of treatment, apart from TCM low-dose group, the lower esophageal mucosa of rats NOS2 expression in other groups were significantly lower than the model group (P <0.05), TCM middle-dose group compared with the model group is not very different, there are no statistics significance (P>0.05).Conclusion:1.After 7days, Xinkaikujiang method for the treatment of RE rats can reduce the esophageal mucosa of rats lower PH value. After14 days, gross morphology and pathological examination of the RE rat can be seen ulcer healing in the lower part of the esophagus mucosa.2.Xinkaikujiang method treatment of RE rats mechanism may reduce NOS2 expression.
