Objective: We would collect venous blood and obtain the dynamic changes of CD4+CD25+CD127-Treg/CD4+T cells ratio(Treg%) in acute phase of acute ischemic stroke patients, know the frequency of regulatory T cells(Tregs) in different infarct volume patients, try to analyze the mechanism underlying the Tregs change in numbers and immunology.Methods: 101 patients diagnosed with acute ischemic stroke who were admitted to the Department of Neurology and the Department of Emergency at the First Affiliated Hospital of Bengbu Medical College were enrolled between June 2014 and November2015. They were divided into small infarct volume group and large infarct volume group based on the cerebral infarct volume being lesser or greater than 30ml(the median infarct volume); 64 age-matched individuals were enrolled as controls who had one or more risk factors for cerebral vascular diseases without stroke. 2m L venous blood would be collected on day 1, day 3, day 7 and day 14 poststroke, only one blood sample was collected from controls on examination day. We would obtain the CD4+CD25+CD127-Treg/CD4+ T cell ratio(Treg%) using flow cytometry after collecting blood in 24 hours. We woule use repeated ANOVA to get the difference between the sex, the cerebral hemisphere and infection or not which has a probable influence on the frequency of Treg cells, thus, we would compare the infected patients and uninfected patients in infarct volume, cerebral hemisphere and cerebral location of different infarct volume groups.Results: 1.Compared to the controls, small infarct volume group showed a slight increase in day 1, and showed a significant increase in day 3, day 7 and day 14(P<0.05).Large infarct volume group showed a significant decrease on day 1 and day 3 and a significant increase on day 7 and day 14(P<0.05).2.There has no difference between men and women in Treg%(P=0.966>0.05).3.There has no difference between the left and right cerebral hemisphere in Treg%(P=0.86>0.05).4.There has no difference between infected and uninfected patients in Treg%(P=0.79>0.05).5.On admission, there are 22 pulmonary infection patients(22%), 15 urinary tract infection(15%), 2 pulmonary and urinary infection patients(2%) and 2 other infection patients(2%); The infection rate of large infarct volume group was significantly higher than the small infarct volume(P<0.05); The cerebral infarct volume of the infected patients was significantly higher than the uninfected patients in these two infarct volume groups(P<0.05); The cerebral location and hemisphere has no difference between the infected patients and uninfected patients in these two infarct volume groups(P>0.05).Conclusions:1.There was a different frequency of CD4+CD25+CD127-Treg/CD4+ T cell ratio in the acute phase of different infarct volume groups, we speculated that regulatory T cells were involved in the immune progress of ischemic stroke and might have a certain immune regulation in occurrence and development of ischemic stroke;2.The level of CD4+CD25+CD127-Treg/CD4+ T cell ratio might be recognized a predictor of the infarct volume in the early stage of cerebral ischemic stroke patients.
