Analysis On Diagnosis And Therapy Of Level â…¡ Lymphatic Metastasis In Thyroid Carcinoma

Objective:Analyze the relevant factors in papillary thyroid carcinoma level â…¡ lymph node metastasis, in order to help decide the region of lymph-node dissection.Method:Retrospectively collect the information of 18 patients in Second Affiliated Hospital of Zhejiang University during April,2014 to April 2015, who are pathologically diagnosed of thyroid carcinoma with level â…¡ lymphatic metastasis. Analyze whether level â…¡ lymphatic metastasis has some relationships with age, gender, size, location, capsule invasion and level â…¥ lymphatic metastasis.Results:In the 18 cases, most are female, below 45 years of age, with larger than 10mm mass, with incomplete capsule, with no significant signs and with no recurrent laryngeal nerve invasion. The number of cases of mass in the superior part, middle and inferior part and diffuse in the thyroid is more or less the same. There exist 2 cases with negative level â…¥ lymphatic metastasis.Conclusion:We should pay attention to the probability of level â…¡ lymphatic metastasis, when the mass size is more than 1cm, thyroid capsule is invaded, or the mass is in the superior part or diffuse in the thyroid. If lateral neck dissection is decided, â…¡ region should be included. And if not, even if lymph-node pathology of VI region is negative, still we should consider the possibility of skip metastasis.
