| Objective: To investigate the clinical significance of middle artery lymph node dissection in radical operation of distal gastric cancer.Method: Patients with locally advanced distal gastric cancer who met the inclusion criteria in the Affiliated Hospital of Youjiang Medical College for Nationalities from August2020 to December 2022 were selected and divided into two groups according to the grouping criteria.In the study group,60 patients underwent laparoscopic radical gastrectomy +D2lymph node dissection + middle artery lymph node dissection before examination.40 patients in the control group received laparoscopic radical gastrectomy +D2 lymph node dissection during the operation.The positive rate of middle colon para-arterial lymph node metastasis,the safety and feasibility of dissection were statistically analyzed.Results: In 100 patients with gastric cancer,98 patients had no enlargement of middle artery lymph nodes,and 2 patients had enlargement of middle artery lymph nodes,both of which were in the study group.Postoperative pathology indicated that 1 patient in the study group had cancer invasion,while the other 59 patients in the study group had no cancer invasion,and the metastasis rate of para-arterial lymph nodes in the middle colon was 1.67%.No intraoperative or postoperative complications related to middle arterial lymph node dissection occurred in the study group.There was no statistical significance in hospital stay,operation time,intraoperative blood loss and postoperative white blood cell count between 2groups(P > 0.05).Conclusions: The metastasis rate of paraarterial middle colon lymph nodes in locally advanced distal gastric cancer is low,and routine dissection is not necessary during radical gastrectomy.If paraarterial middle colon lymph node metastasis is considered according to relevant examinations,lymph node dissection can be performed.Laparoscopic paraarterial middle colon lymph node dissection is safe and feasible. |
