Objective (s):The objective of this research is to evaluate the efficacy and safety of endovascular laser treatment (EVLT) in treatment of venous leg ulcers, and to provide a reliable clinical basis for laser treatment of venous leg ulcers.Methods:There were 60 patients were randomly divided into experimental and control groups, the experimental group of 32 patients and the control group of 28patients.The experimental group was treated with great saphenous vein high ligation combined with EVLT and dressing and elastic bandage treatment; the control group was treated with traditional great saphenous vein high ligation plus ambulatory phlebectomy for varicose vein and dressing and elastic bandage treatment.Compared between the two groups of ulcer healing time, the shrinkage of ulcers, the recurrence rate, hospital stay,pain and other indicators.Results:The average healing time of the experimental group was (18.9± 9.5) days, and the control group was (25.2± 13.0) days, the differential of two groups was statistically significant(P<0.05); The average operative time of the experimental group was (49.4± 6.3) min, and the control group was (61.8± 5.1) min, the differential of two groups was statistically significant(P<0.05); the average blood loss in the experimental group was (16.3± 4.4) mL, and the control group was (36.0± 4.8) mL, the differential of two groups was statistically significant (P<0.05);the average length of hospital stay in the experimental group was (15.7±10.5)days, and the control group was(21.5±13.8)days, the differential of two groups was statistically significant(P<0.05).The average shrinkage of ulcers of the experimental group was(67.4±28.1)%, and the control group was (60.8±16.0)%,the difference of two groups was no statistically significant(P>0.05).Ulcer recurrence rate of the experimental group was 0%, and the control group was 7.1%, the two groups showed no statistically significant (P>0.05).In addition, the incidence of postoperative pain, numbness rate, skin infection rate in the experimental group was significantly lower than the control group (P<0.05); and skin necrosis rate, DVT incidence rates in the two groups showed no statistically significant (P> 0.05).Conclusion(s):Endovascular laser treatment is obviously effctive in the treatment of venous leg ulcers, can speed up venous leg ulcers healing, shorten the hospital stay of patients, reduce the incidence of postoperative limb pain, numbness, skin infections and other complications, and simple operation, less trauma, quicker recovery, beautiful and a more broad application prospects, which is worth of clinical application.
