ObjectiveThe artical is based on the theory of "Huo Yu Fa Zhi", that aims to study the machanism on fire needle treating acute gouty through the Nalp3 inflammasome and IL-1β signaling pathway.MethodsIn this study, sisty Wistar male rats were devided into control group, model group, fire needle group, acupuncture group and colchicine group. Except control group, all other groups were combined with the use of oteracil potassium and Coderre’s acute gouty arthritis model, that builing the HUA and AGA in mice as well as in human beings. This study is based on the theory of "Huo Yu Fa Zhi" to research the AGA’s machanism through the WBC,NALP3 inflammasome and IL-1beta signaling pathway.Results(1) After giving oteracil potassium, the uric acid levels of the rats in all groups went up(p<0.05), except the control group. That meaned the hyperuricemia model was sussessed to make.(2) After building AGA model, all groups’gait, ankle swelling and inflammation index peaked at 24 hours and had statistical difference (p<0.05), except the control group. Post-modeling 24 hours, the fire needle had statistical difference in comparation with the model group on ankle swelling(p<0.05). Post-modeling 72 hours, the fire needle and the needle groups’ankle swelling had statistical difference in compatation with that of the model and colchicine groups(p<0.05). The gait of the fire needle and the colchicine group improved post-modeling 72 hours and had statistical difference(p<0.05) when comparaed with post-modeling 48 hours (p<0.05). But there were no difference in comparation with the model group(p>0.05). The inflammation index of the fire needle and the needle group improved post-modeling 72 hours and had statistical difference when comparing that of the model group(p<0.05). The inflammation index had no difference between the model group and the colchicine group (p>0.05).(3) After AGA modeling, the number of WBC in other four groups increased in comparation with the control group, which had statistical difference (p<0.05). And it decreased significantly after fire neelding and colchicine treatment, which had statistical difference (p<0.05) in comparation with the model group and no difference in these two groups (p> 0.05).(4) After AGA model was made, the serum levels of IL-1β and NALP3 inflammasome were all increased, and had statistical differences in all groups except control group(p<0.05). And the model and the needle group had a significantly statistical difference(p<0.01) for comparison on the levels of IL-1β. In comparation with the model group, the other three treating groups were lower on the levels of IL-1β, which had statistical difference(p<0.05). Post-treatment, the levels of IL-1β decreased significanty in the three treating groups(p<0.05). All groups decreased and there was no difference among them on the levels of NALP3 inflammasome.(5) After HUA modeling, the levels of IL-1β and NALP3 inflammasome in synovial in all groups increased significantly (p<0.05),except the normal group. After AGA modeling, the levels still remained at a high level. But in comparation with the model group, the levels in the other three treating groups were lower(p<0.05). Post treatment, the model group’s IL-1 beta and NALP3 inflammasome levels was significantly higher than the other four groups (p<0.05). And there was no difference among the other four groups on the IL-1β level.But there was statistical difference among the other three treating groups on NALP3 level in comparation with the control group (p<0.05).ConclusionThe research finds out the fire needle could improve the gait and have obviously analgesic and detumescent effect. It may be related to the effect on inhibiting the increases of white blood cells, the activation of NALP3 inflammasome and IL-1beta signaling pathway.
