Analysis Of Risk Factors On Lung Metastasis In Differentiated Thyroid Carcinoma

Objective To investigate risk factors of lung metastasis in differentiated thyroid carcinoma (DTC)Methods a total of 335 case of DTC treated at the First affiliated hospital of Guangxi Medical University from 2010 to 2015 were collected retrospectively. The diagnosis standard of DTC lung metastasis include 131I whole-body scan, CT, or lung nodule pathology.43 patients were diagnosed as DTC lung metastasis. The clinical pathological data includes greatest tumor diameter, single focal or multifocal tumor, thyroid capsular invasion, thyroid neighbor tissue invasion, the number of neck lymph node metastasis, Analyzed the risk factors in lung metastasis of DTC, and the correlation analysis including singular and multiple factors was conducted with logistic regression method.Results 43 lung metastasis in 335 DTC patient. Univariate analysis reveal that histopathological type(P=0.004), tumor diamete(P=0.001), multifocal tumor(P=0.001), thyroid capsular invasion(P=0.002), the number of neck lymph node metastasis(P=0.001) are relevant to DTC with lung metastasis, age(P=0.307) and gender(P=0.515) are not relevant to DTC with lung metastasis. Logistic regression analysis show that the factors such as tumor diameter> 2cm(P=0.024, OR=1.258-7.135), thyroid capsular invasion(P=0.018, OR=1.141-12.552), the number of neck lymph node metastasis>5(P=0.023, OR=1.024-5.231) are risk independent factors for lung metastasis of DTC.Conclusion The DTC patient with tumor diameter> 2cm, thyroid capsular invasion as well as the number of neck lymph node metastasis>5 may be more likely to undergo lung metastasis of DTC. It reveals that DTC lung metastasis is closely related to clinical pathological features. So we should pay attention to these clinical pathological features, it is benefit to diagnose DTC with lung metastasis earlier.
