The Analysis Of Risk Factors For Differentiated Thyroid Carcinoma With Bone Metastasis

Objective: To analyze the risk factors of bone metastases in patients with DTC.Methods: From June 2012 to March 2017 in 960 cases of DTC patients in the First Affiliated Hospital of Guangxi Medical University Department of nuclear medicine,31 cases in bone metastasis group,according to random sampling method in 119 cases of non bone metastasis group,a total of 150 patients in the present study.Finishing patients' gender,age,time of diagnosis of bone metastasis,thyroid(surgery way,cancer foci,postoperative pathological types),cervical lymph node(dissection,metastasis),lesion biopsy or postoperative pathological types of bone,the first time to thyroidectom after the operation.X~2 test was used for univariate analysis and Logistic regression wasused for multivariate analysis to investigate the risk factors of bone metastasis.Results: The incidence of DTC with bone metastasis is 3.2%(31/960).Univariate analysis: age(X~2=10.554,P=0.005),time of diagnosis of bone metastasis(X~2=7.274,P=0.026),unilateral or bilateral thyroid cancer foci(X~2=15.444,P=0.000),postoperative pathological types(X~2=7.688,P=0.006),neck dissection(X~2=6.072,P=0.048),the first time to thyroidectom after the operation(X~2=29.408,P=0.000)are associated with bone metastasis;Logistics regression analysis: Logistics(P)=-6.487-4.067(age 20-45 years old)+4.236(bilateral thyroid cancer foci)-5.126(non neck dissection)+3.877(the first time to thyroidectom after the operation 6-12months)+3.921(the first time to thyroidectom after the operation >12 months)+3.858(the first time to thyroidectom after the operation is unknown).Conclusion:20-45 years old,bilateral thyroid cancer foci,non neck dissection,the first time to thyroidectom after the operation ?6 months are the exact risk factors for bone metastasis of DTC.
