| BackgroundCancer patients with cancer pain, mainly refers to the invasion of tumor cells, proliferation, metastasis or oppression caused by the relevant organizations, is the highest incidence of many symptoms. Although there are three step analgesic ladder internationally recognized approach to the treatment of cancer pain, but failed to do all the pain to a rational state control. Acupoint catgut embedding therapy has analgesic effect, stimulating effect, long without repeated operation advantage. In order to further explore the method of treating cancer pain better, so we speculated according to spinal nerve segment distribution of Beishu acupoint catgut embedding therapy has a role in the treatment of cancer pain.Object i vesThis topic will be from a clinical point of view through to cancer patients in the three step analgesic ladder method based on, combined with spinal ganglion segment distribution of Beishu points, and only three ladder analgesic method carries on the curative effect of contrast, to ease the pain of two groups and the quality of life, the Hospital Anxiety and depression, acupoint catgut embedding Beishu Points in the treatment of cancer pain provide basis for the clinical assessment.MethodsThe cohort study design principles by four criteria:diagnostic criteria, inclusion criteria, exclusion, falling standards, in the First Affiliated Hospital of Guangzhou University of traditional Chinese medicine observed inpatient cancer center in 60 patients, patients with moderate to severe cancer pain, (according to the patient’s wishes is divided into the experimental group (30 cases) and control group (30 cases) and the control group were treated with "three ladder analgesic method" to medicine, NRS score of 4 to 6 points, belonging to moderate pain, the use of drugs for tramadol hydrochloride sustained release tablets (100mg/piece),1 times/piece, q12h; NRS score 7 to 10 points, which belongs to the severe pain, the use of drugs for hydrochloric acid by oxycodone controlled-release tablets (10mg/piece),1 times/piece, q12h, or morphine sulfate controlled release tablets (30mg/ piece),1 times/piece, q12h, or fentanyl transdermal system respectively), 1 times/stickers, Q72H If the control is not ideal, according to the degree of pain relief to be dose titration, until satisfactory. The test group is on the basis of this, coupled with the spinal ganglion segment distribution of Beishu points catgut embedding,1 times a week. And recorded daily in the NRS score, seven days before treatment and after treatment given E0RCT-QLQ-C30 scale to assess the quality of life of patients, and hospital anxiety and Depression Scale (HADS scale) to evaluate the change of the 7 days in patients with cancer pain objective pain before treatment and after treatment.ResultCompared with the control group, the rate of pain relief was statistically significant (P<0.05). Analysis showed that the "experimental group" in third days can be better to ease the severe pain.After 7 days, evaluation of the quality of life of the two groups, compared with before treatment, scores of the function in the field of physical, cognitive, social and main symptoms in the field fatigue, pain, insomnia and loss of appetite are improved, with significant difference (P<0.05). In the 7th day after treatment, the experimental group and control group of quality of life (QOL) table score comparison can be seen both in the main symptoms in the field and in the experimental group, fatigue and insomnia is better than the control group, with significant difference (P<0.05), while the remaining such as general health status of sub scale, functional areas and most of the major symptoms in the field, there is no statistical difference (P>0.05).Before and after treatment with HADS self rating scale evaluation, this study shows that after the treatment of anxiety and depression were better than those before treatment, with statistical difference (P<0.05).Conclusion1. To spinal nerve segment distribution Beishu acupoints catgut implantation at acupoint combined with three ladder analgesic method can improve the efficiency, show Beishu acupoints catgut implantation at acupoint combined with three-step group may have a comparative advantage.2. The side effects of the spinal ganglion segment distribution to select Beishu acupoints catgut implantation can reduce the pain of the three step method.3. Can improve the quality of life of patients. The anxiety and depression of4 patients with cancer pain, effective analgesia can relieve the patient’s anxiety and depression. |
