Objective:(1)To investigate the relative risk factors of the disseminated intravascular coagulation(DIC)with malignant solid tumor;(2)To investigate the difference between the disseminated intravascular coagulation with malignant solid tumor and the DIC with acute leukemia. Methods:(1)The clinical data of 54 malignant solid tumor patients(observation group) with DIC from January 2004 to December 2014 were analyzed retrospectively. And 54 malignant tumor patients were analyzed;(2) The clinical data of 70 acute leukemia patients with DIC from January 2004 to December 2014 were analyzed retrospectively, and is compared with the malignant solid tumor patients with DIC. Results:(1)Multiple factor analysis showed that advanced tumor(OR=0.252,P=0.019)、complicated with hypoproteinemia(OR=0.119,P=0.005)、complicated with infection(OR=0.122,P=0.003) were the independent risk factors to the malignant solid tumor patients with DIC;(2)The mortality of malignant solid tumor patients with stage I/II in observation group was higher than that in control group(χ2=15.40,P<0.001); the mortality of malignant solid tumor patients with stage III/IV in observation group was higher than that in control group(χ2=42.42,P<0.001);(3)The number of bleeding performers in the malignant solid tumor patients with DIC is 14(31.5%),and most of them are the hemorrhage of digestive tract(16.7%).The number of bleeding performers in the group of acute leukemia patients with DIC is 52(74.3%),and most of them are the mucocutaneous hemorrhage;(4)The differences between malignant solid tumor patients with DIC and acute leukemia patients with DIC are PLT(Z=-5.773,P<0.001)、PT(Z=-4.571,P<0.001)、APTT(Z=-4.763,P<0.001)、FIB-C(Z=-4.301,P<0.001)and hypoproteinemia(χ2=21.969,P<0.001);(5)The mortality of malignant solid tumor patients with DIC was higher than the group of acute leukemia patients with DIC(χ2=47.369,P<0.001). Conclusion:(1)Advanced tumor、hypoproteinemia、 and infection are the risk factors for DIC in patients with malignant tumor;(2)The mortality of malignant tumor patients with DIC is high.(3)The main clinical manifestation of malignant solid tumor patients with DIC is the hemorrhage of digestive tract; the main clinical manifestation of acute leukemia patients with DIC is mucocutaneous hemorrhage.(4)The counts of PLT、PT、APTT、FIB-C of malignant solid tumor patients with DIC are higher than the group of acute leukemia patients with DIC; the number of patients with hypoproteinemia in the group of malignant solid tumor with DIC are higher than the group of acute leukemia with DIC.(5)The mortality of malignant solid tumor patients with DIC was higher than the group of acute leukemia patients with DIC.
