Abstract/Summary:
Objective:To analyze the impact of chorionicity on pregnant women and peri nates, in order to draw clinicians’attention to chorionic diagnosis and provide t win pregnancy with assistance about pregnancy management.Methods:1. Materials:177 twin pregnant women visiting Maternity Ward of Zhongda Hospital affiliated to Southeast University from August 2006 to December 2014 were recruited for the study.126 twin pregnancies received diagnosis of chorionicity, including 88 DCDA twin pregnancies,35 MCDA twin pregnancies and 3 MCMA twin pregnancies. Retrospective analysis of the clinical data of twin pregnancies was done to analyze the impaction of chorionicity on pregnant women and perinates.2. Statistical methods:Use SPSS 19.0 software to analyze the data. Measurement data use t test, count data use χ2 test. If T< 1, or N<40, or after χ2 test, P is close to the test level α, use Fisher’s exact test; if 1≤T<5 and N≥40, use correction χ2 test. P< 0.05 was considered statistically significant.Results:1. Impact of chorionicity on the pregnant women:Among the premature labour, PROM, PIH, ICP, GDM, polyhydramnios, Anemia, placenta praevia, placental abruption, postpartum hemorrhage and the mood of delivery, There was statistically significant difference in premature labour and ICP between two groups (P<0.05), but there was no statistically differences in other complications.2. Impact of chorionicity on the fetus:Among one fetal death, IUGR, inconsistent twins, malformation and fetal distress, There was statistically significant difference in inconsistent twins and malformation between two groups (P<0.05). MCDA group had 7 cases complicated by twin-twin transfusion syndrome and the morbidity was 20%.3. Impact of chorionicity on the newborns:The gestational age and birth weight in MCDA group were significantly lower than those in DCDA group(P<0.05). Asphyxia, occupancy of NICU and the incidences of RDS, HI, PDA in MCDA group were significantly higher than those in DCDA group(P<0.05). There were no significantly diference in the incidences of neonatal death, septicaemia, hyperbilirubinemia, hypoglycemia and so on between MCDA group and DCDA group (P>0.05). The incidence of asphyxia and neonatal death will decrease with increasing gestational age and birth weight.Conclusions:The study shows that:1. MCDA twin pregnancy is a risk factor for preterm birth and ICP.2. Chorionicity is closely related with the complication and perinatal outcome of perinatal infants. The prognosis of MCDA twin pregnancy is significantly worse than prognosis of DCDA twin pregnancy.3. The incidence of asphyxia and neonatal death is negatively correlated with gestational age and birth weight.
