ObjectiveTo investigate the hips and lower limb fractures in elderly patients with cognitive function and nutritional status.By nutritional cognitive intervention, evaluation the application effect on elderly fractures patients and compare the different effect of nutritional comprehensive intervention in different cognitive level on elderly fractures patients.MethodsTotally 80 fractured elderly( > 60 years old)who visited our hospital between Nov.2014 to Jun. 2015 were enrolled. Their cognitive function level, nutritional status and activity ability were studied with Montreal cognitive assessment scale, Mini nutritional assessment scale and Daily activity ability scale.According to Mo CA,divided into two groups.After nutritional comprehensive intervention, three months after discharged from hospital they were again investigated. Evaluation the application effect on elderly fractures patients and compare the different effect of nutritional comprehensive intervention in different cognitive level on elderly fractures patients.Results1. The incidence of fracture of elderly patients with mild cognitive dysfunction in hospital was 48%, the incidence of malnutrition and risk of malnutrition was 58%.2. The cognitive function including visual space,execution capability, name,note,delayed memory and directional force was positive correlation with nutritional status(r=0.322、0.419、0.251、0.215、0.237、0.350,P<0.05); Language and abstract thinking was no correlation with nutritional status.3. After the intervention, nutritional status and daily activities ability were higher than before, while conflict were at a high level(P<0.05). No serious complications and death, function were gradually recovered and achieve nursing desired effect.4.After the intervention,the change of nutritional status of normal cognitive function group was higher than mild cognitive impairment group, the overall evaluation anddietary change difference is statistically significant(P<0.05).There was no statistically significant difference in human body measurement and subjective evaluation(P<0.05).5. After the intervention,the change of daily activities ability in normal cognitive function group was higher than mild cognitive impairment group, the ability of physical Self-maintenance and the ability of instrumental activity of daily life are statistically significantly different(P <0.05).Conclusion1. The incidence of malnutrition and mild cognitive impairment in the hips and lower limb fractures in elderly patients are higher. The differernt levels of cognitive function in patients have different nutritional status, and the better cognitive function, the better accepting nutritional comprehensive intervention. Therefore, Medical staffs should pay attention and take corresponding measures in time to improve the level of cognitive function and nutritional status and pay attention to in the screening and training of patients with mild cognitive impairment and ensure those patients can effectively accept and comply with nursing intervention measures.2. Nutritional comprehensive intervention can promote fractures in elderly patients’ nutrition status and functional recovery. Therefore while we corespond rehabilitation plan,we should put nutrition intervention as an important contant.
