Objective:To explore the risk factors of pulmonary fungal infections(PFI) and provide evidences for its clinical effective prevention and treatment.Methods:175 hospitalized patients in the department of respiration of our hospital from January2011 to June 2015 were divided into two groups according to their discharge diagnosis : 86 cases of PFI and 89 cases of non-PFI.Collect the clinical data including basic information,basic diseases,invasive operations, broad-spectrum antibiotics,glucocorticoid,immune inhibitor use and other factors.Single factor analysis method was used to select the risk factors and multivariate logistic regression analysis method was used to determine the independent risk factors of PFI.Results:1.Single factor analysis showed:The length of hospital stay(P =0.047), smoking history(P =0.027),chronic obstructive pulmonary disease(P =0.018),indwelling catheter(P =0.002), central venous catheterization(P =0.023) and mechanical ventilation(P=0.034), long duration of corticosteroid use > 2 weeks( P =0.001), long use of broad-spectrum antibiotics > 2 weeks(P <0.001),low protein hyperlipidemia(P =0.015)were related to the presence of PFI.2.Multivariate logistic regression analysis showed:indwelling catheter(P =0.044,OR=3.142),long duration of corticosteroid use > 2 weeks(P =0.012,OR =3.369),long use of broad-spectrum antibiotics > 2weeks(P =0.005,OR =3.138)and low protein hyperlipidemia(P =0.018,OR =2.389) were the major risk factors for the occurrence of pulmonary fungal infection.Conclusions:The long term use of corticosteroid and broad-spectrum antibiotics,indwelling catheter and low protein hyperlipidemia may be the independent risk factors of pulmonary fungal infection.
