Retrospective Study On The Efficiency And Safety Of Clopidogrel In Patients With Acute Cerebral Infarction

ObjectiveTo study the effects and safety of the use of Clopidogrel in patients with acute cerebral infarction.MethodThis retrospective study is based on a single center. We collected the clinical materials of acute ischemic stroke patients within 7 days of onset of the stroke in the 1st Affiliated Hospital of Jinan University from January 1, 2012 to May31,2015, including the basic information,cardiovascular risk factors, etiology classification of cerebral infarction, antiplatelet drugs use, the NIHSS score in Chi-squares test was used in statistical and scientific analysis.ResultFrom January 1st, 2012 to May 31 st,2015. There is a total of 1008 cases.There were 94 cases with progressive stroke. Within 7 days from the onset of stroke, clopidogrel group including basic and loading clopidogrel groups show no advantage to aspirin in reducing the rate of early neurological recurrence and deterioration. within 24 hours from stroke onset,clopidogrel is superior to aspirin in reducing early neurological recurrence and deterioration.However, in groups of NIHSS >3 and NIHSS 8-15,clopidogrel especially initially load and routine clopidogrel is superior to aspirin in reducing early neurologicalrecurrence and deterioration. In acute cerebral infarction,there are 2 cases included in the primary safety endpoint and 38 cases in the secondary safety events.Difference among the aspirin,clopidogrel including routine clopidogrel,initially load and routine clopidogrel groups,There is no significance for the rates of secondary safety event.ConclusionThe using of clopidogrel is safe in acute phase for infarction patients in routine or nitially loading and routine clopidogrel groups.Within 24 hours of onset of acute cerebral infarction patients, clopidogrel can decrease the incidence of early neurological recurrence and deterioration more than aspirin.The use of clopidogrel especially initially loading and routine clopidogrel can decrease the incidence of early neurological recurrence and deterioration for patients of NIHSS>3 points.
