ObjectiveTo investigate the correlation between dual qi-yin deficiency and yang deficiency of spleen and kidney on expression of inflammatory factors and CysC in early renal injury patients with T2 DM, reveal the distribution of syndromes between Cys C and inflammatory factors,in order to explore the risk factors of the early renal injury patients with T2 DM.MethodSelect two main groups according to TCM syndrome types in early renal injury patients with T2 DM, group dual qi-yin deficiency(group NA) and group yang deficiency of spleen and kidney(group NB),and 40 patients in each group were collected. Other 40 healthy cases were chosen as control group over the same period(group NC).CysC、MCP-1、IL-6、TNF-α level were detected by ELISA.FPG、2hPG、HbA1c、renal function and blood lipids level were measured as well.Results1. Indexes analysis of the three groups:(1)Compared with group NC, the FPG, 2hPG, HbA1 c, 24h-mAlb, TG, LDL-C, and HDL-C level of group NA and group NB,which differences were statistically significant(P<0.05); But the difference between the group NA and group NB was not statistically significant(P>0.05);NB group, 2hPG, HbA1 c was no significant difference(P>0.05);(2)There was not statistical difference in BUN and Scr among these 3 groups(P>0.05);(3)There was statistical difference in CysC,MCP-1,IL-6 and TNF-α between all syndrome groups and control group(P<0.05), besides, compared with the group NB, the CysC, IL-6, TNF-α, MCP-1 levels of group NA were significantly increased(P<0.05);2. Indexes correlation analysis between Two syndromes :(1)In the group NA, the results demonstrated a negative correlation between CysC and IL-6(r=-0.514,P<0.01), CysC and FPG(r=-0.514,P<0.01), TG and HDL-C(r=-0.514,P<0.05),and a positive correlation between TG and FPG(r=0.378,P<0.05),TG and HbA1c(r=0.511,P<0.01),2hPG and FPG(r=0.557,P<0.01);In addition, regarding CysC as dependent variable in group NA, the independent variable- IL-6 was statistically significant(B=-0.006, P<0.05).(2)In the group NB, TG and LDL-C, 2h PG and FPG, Hb A1 c and HDL-C were positively correlated(r=0.467, P <0.01)(r=0.575, P<0.01)(r=0.466, P<0.01),but there was no significant correlation between other indexes(P>0.05).Conclusion1.Cys C, IL-6, TNF-α and MCP-1 play important roles in type 2 diabetes with early kidney damage, especially in dual qi-yin deficiency; 2. There may be a cause-and-effect relationship between the IL-6 and Cys C,and the IL-6 might be a risk factor of qi-yin deficiency in early renal injury patients with T2DM; 3.Blood lipid and glucose plays an important role both in dual qi-yin deficiency and yang deficiency of spleen in early pathogenesis of renal injury patients with T2 DM. |
