Objective :Currently, scar treatment methods mainly include corticosteroid injection,surgical resection, radiation therapy, pressure therapy and so on. The above methods work but not very well. The reason of the scar forming mechanism was not clear. The mucosal healing test demonstrated that inflammation was minimal,heal scar was light,and the mucosal wound was fast. Therefore, the study, through the content measurement of MMP-1and TGFβ1 in lip skin and mucosal scar and the differences of MMP-1 and TGFβ1in lip mucosal and skin scar, discussed whether the light healing mechanism of lip mucosal scar was related to the content of MMP-1and TGFβ1, which can provide a theoretical basis for scar treatment.Methods:This study totally selected 30 cases among the harelip patients with lip skin excised in the phase Ⅱ reconstructive surgery and mucosal scar from March 2015 to October 2015 at the Oral and Maxillofacial Surgery of Qingdao Hiser Medical Group. The cases was divided into two groups, A group and B group and every group had 15 cases. The morphological difference in histology of lip skin and mucosal scar, observed through the routine immunohistochemical staining on the MMP-1and TGFβ1 of the scar tissue were used to compare the protein expression level differences of MMP-1 and TGFβ1 in the scar tissue of the two groups. Five horizons under 200 magnification lens was randomly selected. Through imagepro-plus professional image analysis software, the integrated optical density(IOD) of MMP-1 and TGFβ1 and regional area was measured, the average optical density value(AOD) =IOD/area reflected expression changes of MMP1 and TGFβ1protein, and then the average value can be taken. The data of the two groups, by comparative analysis of data statistical software SPSS 20.0, compared the content differences of MMP1 and TGFβ1 in lip skin and mucosal scar.Results : The TGFβ1 content of the lip skin scar tissue(0.1096 ± 0.0159) obviously increased in comparison with that of the lip mucosal scar tissue(0.0359 ± 0.0104), the positive expression was relatively strong, and P < 0.05 has differences in statistics. The MMP1 content of the lip mucosal scar tissue(0.1093 ± 0.01266) increased in comparison with that of the lip skin scar tissue(0.0428 ± 0.0064), the positive expression was relatively strong, and P < 0.05 has differences in statistics.Conclusion: 1.In histology, there are obvious differences in morphological structure after harelip operation between the lip mucosal and skin scar under the microscope. Keratinocyte cell layer of lip skin scar was relatively thick, papillary layer almost disappeared, the proliferated fiber connective tissue in dermis increased, collagen was thick, big and messy in the shape of vortex and rope, and blood capillary was abundant but part of the lumen narrowed and even closed. Mucosa scar nail was long and narrow, there were more blood capillaries than skin, mainly on the papillary layer, and dermis collagen was in an order.2. The TGFβ1 protein expressed strong positive in the lip skin scar and MMP1 protein expressed weak positive, but the lip mucosal scar was on the contrary, which reveals that the light healing lip mucosa scar may be related to TGFβ1 and MMP1.3.The expression level of TGFβ1 and MMP1 protein was different in lip skin scar and mucosal scar after the operation of cleft lip.
