Expression Of MMP-1 And TIMP-1 In Hypertrophic Scar And Normotrophic Scar Of Skin Tissue After Ceft Lip Surgery

Objective: To detect expression of MMP-1 and TIMP-1 in the upper lip scar tissue after postoperative of cleft lip repair and study the correlation between the two index,discussed the expressed differences of MMP-1 and TIMP-1 in scar tissue after surgical repair treatment of cleft lip,Further understood the formation mechanism of scar,and to found theoretical foundation in the treatment of scar tissue in upper lip after the postoperative of cleft lip repair.Methods: Total samples are collected from the patients which are from postoperation of unilateral cleft lip repair and debridement of upper lip trauma of the oral and maxillofacial surgery in the Affiliated Qingdao Hiser Medical Center of Qingdao University Medical College,22 samples hypertrophic scar(HS)and 18 normotrophic scar(NS)samples after the second cleft lip repair,15 samples normal skin tissue that were abandoned after debridement and repair of upper lip trauma.It was used to observe the morphology and collagen arrangement of fibroblasts in the scar tissue by HE staining;the expression of MMP-1,TIMP-1 in different tissue was observed by immunohistchemical staining;Through the Image-Pro Plus professional Image processing software,the integrated optical density(IOD)and area from each picture were measured,and the average optical density(AOD)=IOD/area.The value of AOD was regarded as an index of the expression content of MMP-1 and TIMP-1;By SPSS 20.0 statistical analysis,the average of AOD in each groups were obtained by dates processing.Compared with the expression content of MMP-1,TIMP-1,if there were found statistically significance between them.Results: 1.Observed under the microscope:(1)Normotrophic scar tissue: The corneum of the epidermis was thicker,and there were seen skin adnexals,such as sweat glands,hair follicles;in the dermal lay,there were found the increased fibrillar connective tissue,bulky collagen fibrils which were arranged in order and some inflammatory cells.(2)Hypertrophic scar tissue: The corneum of the epidermis was thicker and the disappeared papillaris pars;in the dermal lay,there were found the increased fibrillar connective tissue,bulky collagen fibrils which were arranged in disorder and shaped in spirals and cords,and increased cappillary.2.The expression content of MMP-1 in normal skin group after trauma,HS group,and the NS group after the second repair of clef lip were 0.023657�0.008477,0.040395�0.011534,0.097905�0.020593,the expression of MMP-1 in HS group of upper lip was highest in the three groups and the normal skin group of upper lip was lowest;which compared with the random two groups in three groups compared,there were found statistically statistically significance between them(P<0.05);3.The expression content of TIMP-1 in normal skin tissue of cleft lip,NS tissue,HS tissue after the second repair of clef lip were 0.024574�0.009210,0.053827�0.015087,0.157606�0.031090.The expression content of TIMP-1 in the HS tissue was highest in the three groups and the normal skin tissue was lowest;which compared with the random two groups in three groups compared,there were found statistically statistically significance between them(P<0.05).Conclusion: 1.There were some differences in quantity and arrangement of collagenous fibers in the tissue of HS and NS after the postoperative of cleft lip repair,and they are different from the normal skin in cutaneous appendages and quantity and arrangement of collagenous fibers.2.It manifested that the formation of the skin scar of upper lip could be regulated and controlled by MMP-1 and TIMP-1.3.The study showed that it could be one of the reasons related to severity of scar formation,that were differences between the expression of MMP-1and TIMP-1 in the HS skin tissues and the NS skin tissue of upper lip after unilateral cleft lip repair.
