| Objective:By collecting the 4 years’ experience of myocardial protection in pediatric cardiac surgery of TOF, we compare the myocardium protective effect of HTK solution versus modified STH solution and the differences of single-dose perfusion and double-dose perfusion of modified STH solution. We explore the appropriate approach in myocardial protection strategy in order to improve the protection of immature myocardium.Materials & Methods:77 pediatric patients of TOF who received the total surgical repair were reviewed, from January 2014, the time HTK solution was firstly used in our hospital, to October 2015. A horizontal comparison between HTK solution and modified STH solution has been made since the HTK solutions were started to use in our hospital. The patients were divided into the HTK group(n=35), STH group(n=33). The perioperative values of the groups were analyzed in present study. The self-cover time, ICU stay, overall stay, Mechanical Ventilation post-operation, post-operation stay, overall stay and perioperative echocardiographic results were analyzed in this study. A vertical comparative method has been adopted to evaluate the effect of single-dose and double-dose perfusion of modified STH solution from October 2011 to October 2015. The patients used modified STH solution were divided into the single-dose group(n=67) and the double-dose group(n=18) as the ACC times were between 56 min to 96 min. The preoperative, intraoperative and postoperative date were analyzed.Results:Self-cover time was significantly statistically shorter in HTK group(0.26min±0.56) than that in STH group(1.33min±1.02, P<0.001). No statistical differences were found between the two groups in ICU stay(p=0.29), postoperative mechanical ventilation time(P=0.84), overall stay(P=0.73). Furthermore, no statistically significant differences were found in perioperative values between the single-dose group and double-dose group.Conclusion:Aimed at pediatric cardiac surgery of TOF, this study suggests that with similar aorta cross clamping(ACC) time, modified STH solution is as safe as HTK solution. The single-dose perfusion may be as efficient as the double-dose perfusion of modified STH solution in a limited ACC time. |
