Objectives To observe and analyze the clinical effect of oxcycodone hydrochloride injection in patients undergoing thoracic surgery with patient-controlled analgesia.Methods 190 patients who had taken thoracic surgery in Tangshan People’s hospital ’s department of tharacic surgery and agreed on the postoperative analgesia were selected,while 183 patients were selected as research objects according to the inclusion and exclusion criteria. Patients were randomly devided into three groups as oxcycodone hydrochloride injection( Group Q, n=61), dezocine injection group(Group D, n = 61) and sufentanil injection group(Group S, n = 61). All patients did not use sedative drugs before operation, and the routine intravenous anesthesia was performed. 30 minutes before the end of the operation, a loading dose of PCIA( 0.1mg/kg oxycodone injection in group Q, 0.1mg/kg dezocine injection in group D, 0.2μg/kg sufentanil injection in group S) was administered to patients. The PCA pump was connected to patiens as soon as the surgery was finished with a background infusion at 2ml/h, a bolus dose of 0.5ml and lockout interval 15 min, and the drug dosage ratio of each group was as follows,oxycodone hydrochloride injection 0.8mg/kg and tropisetron 5mg with 0.9% normal saline diluted to 100 ml was in group Q, dezocine injection 0.8mg/kg and tropisetron 5mg with 0.9% normal saline diluted to 100 ml was in group D, sufentanil injection 2.5μg/kg and tropisetron 5mg with 0.9% normal saline diluted to 100 ml was in group S. The awakening time, extubation time,the mean arterial pressure(MAP), heart rate(HR),Oxygen saturation of blood at the time point of before extubation, tracheal extubation,15 min after the extubation were recorded by staff who was ignorant of the research after patients were sent to the post--anesthesia care unit(PACU) at the end of the operation.Postoperative pain was recored at 2h, 4h, 12 h, 24 h and 48 h after patients returend to the wards, and postoperative analgesic effect were estimated by Visual Analogue Scale(VAS)and Ramsay Sedation Scores(RSS), otherwise the adverse effects, the number of attempts and successful deliveries were recorded in 48 h.Results 1 Compared with group S, the awakeing time and extubation time were shorter in group Q and D, the difference was statistically significant(P<0.05). 2 Patients in S group showed a lowest MAP, HR, Sp O2 at three time points in the extubation period and the difference was statistically significant among three groups(P<0.05), and group Q showed a lower MAP, HR at tracheal extubation than group D(P<0.05). 3 The comparison of cooperation scores in awakeing period in group D was highest among three groups, the difference was statistically significant(P<0.05). 4 Compared the analgesic scores in different time points with three groups, there was no significant difference in Q group and S group, and they showed a lower scores than group D(P>0.05). 5 The effect of sedation with sufentanil injection is superior to the other two(P<0.05). 6 Compared with D group and S group, there was lower incidence of nausea and vomiting in the Q group(P<0.05).7 The number of attempts and successful deliveries within 48 h, there was no no significant difference between group Q and Q(P>0.05), and both of them showed a less number of attempts and successful deliveries than group Q(P<0.05).Conclusions 1 30 minutes before the end of the operation, 0.1mg/kg oxycodone hydrochloride injection can help patients undergoing thoracic surgery receive a best successful awakening period among 0.1mg/kg dezocine injection and 0.2μg/kg sufentanil injection. 2 The effect of analgesia with 0.8mg/kg oxycodone hydrochloride injection is similar to 2.5ug/kg sufentanil injection, while they are better than 0.1mg/kg dezocine injection, otherwise, the oxycodone hydrochloride injection has lowest incidence rate of adverse reaction.
