| Objective: Postoperative pain will increase postoperative complications and is one of the important factors to delay the recovery of patients.Spinal surgery is traumatic,the wound is easy to be compressed after the operation,resulting in severe postoperative pain.The patient-controlled subcutaneous analgesia pump(PCSA)can be used for postoperative analgesia of spinal surgery.In this paper,the sedative and analgesic effects and adverse reactions of sufentanil,desocine and hydromorphone respectively applied to postoperative PCSA of patients undergoing spinal surgery were investigated,so as to provide a basis for the selection of PCSA medication.Methods: A total of 120 patients who underwent spinal canal lesion resection un der microscope and underwent PCSA in the First Affiliated Hospital of China Medical University were selected and randomly divided into group A,group B and group C(all 40 cases per group).Group A: Sufentanil 3~4 ug/kg+ dexamethasone 10 mg+lidocaine 200 mg.Group B: dizocine 0.4~0.5 mg/kg+ dexamethasone 10 mg+lidocaine 200 mg.Group C: hydromorphone 0.4~0.5 mg/kg+ dexamethasone 10mg+ lidocaine 200 mg.Visual Analogue Scale(VAS)was used to record subjects’ pain scores,and Ramsay score was used to record sedation satisfaction rate.Vital signs,analgesia score,sedation satisfaction rate,adverse reactions and additional compressions were recorded 4,16,24 and 48 h after surgery i n 3 groups.Results: There was no significant difference in baseline data among the three groups(P > 0.05).In the application of sufentanil,desoxocin and hydromorphone in postoperative PCSA analgesia of patients undergoing spinal surgery,there were no significant differences among the three groups in the aspects of vital signs,sedation and analgesia scores,pump pressure times and the incidence of adverse reactions 4,16,24 and 48 h after surgery(P > 0.05).As for the satisfaction rate of sedation,the satisfaction rate of sedation in group A,B and C was 100%(40/40),82.50%(33/40)and 85.00%(34/40),respectively.The satisfaction rate of sedation in group A was higher than that in group C and C was higher than that in group B,with statistical significance(χ2=7.419,P= 0.025 < 0.05).The scores of nausea and vomiting in group A were lower than those in group C,and those in group C were lower than those in group B,with statistical significance(P < 0.05).Conclusion: Sufentanil 3~ 4ug /kg has a good analgesic and sedative effect on postoperative PCSA in patients undergoing spinal surgery.Compared with dezocine and hydromorphone,sufentanil has little adverse reactions to nausea and vomiting,and the degree is less,the price is relatively low,the patient’s satisfaction rate of sedation is higher,and thus the application scope is wider. |
