In the complicated situation of the current world, while bioethics issue is becoming increasingly urgent, military medical ethics is trapped in the embarrassment of implementing in wartime. Military and medical are facing the acute conflict in the battlefield. With the Chinese international status rising and rising, Chinese soldiers participate in international military operations, like overseas defense, the maintenance of strategic channel, joint military exercise etc., more frequently. What principle should we follow when encounter the military medical ethics problem in medical works in battlefield? What kinds of problems need us to know and do the research? This is the topic we concerned in this text. Combatants need the clear perfect policies, or guideline principle to properly solve the ethics problems encountered in wound treatment. Domestic military medical ethics research started relatively late, research power is weak, which is not adapted to and the development of practice and the needs of situation. Existing military medical regulatory documents have paid less attention to ethics, its provisions for some controversial problems is relatively obsolete. Under this background, combining with the international heated topics, drawing comparison of foreign military, integrating with Chinese actual situation, and in-depth analyzing on the key issues of medical ethics of wound treatment, which is very important for our army medical work. This article takes the dialectical materialism and historical materialism as a guide, in the literature and the historical facts based, uses the research method like empirical analysis, qualitative research, comparative analysis, summarizes etc. what’s more, it distributed the questionnaire to our army and local health workers to collect their opinions of the ethics problems in wartime wound treatment, including the triage of wound, the informed consent right of war, the right of privacy, and the right to self-determination of wound combatants, and to know their ethical cognitive levels. Then, we combined with the investigation data analysis result, carried on the discussion and research on these problems, hope to draw beneficial enlightenment for our military, and puts forward some suggestions on the developments of our military medical ethics research and policies.This paper describes the background, purpose and significance, introduces the research methods and technical route in the introduction part.The main content is divided into four parts.The first part is the literature review. In this part, for several issues discussed in this article, the author collected the related literature at home and abroad, and analyzed retrospectively, summarized the ideas and views on domestic and foreign related issues, then, found out the deficiencies of domestic research in this aspect.The second part is the survey of cognitive level on ethical issues in wartime wound treatment. In this work, the author picked up 139 people, including office working personnel, research managers, medical researchers and doctors and hospital administrators, to investigate the cognitive level of wartime wound treatment work executives, managers and rulemaking staff of triage, the right of informed consent the right to privacy, the right to self-determination of wound soldiers, battlefield euthanasia and palliative care, the degree of popularization of 2006 edition of “the rules of wound treatment in battlefield”, and people’s view on the triage principle and method in which. Then, the author statistically analyzed the data, and found that the average cognition level of our army and local health workers on related ethics problems is low, and because of the lack of guide, many people have the fuzzy and contradicted ethics view. The cognitive level of different occupation groups have significant difference(P<0.05), which proved that popularity and understanding of regulations and principles and the practice of work are important to improve the ethical cognition and promote relevant research.The third part is the discussion of medical ethics problems in wartime wound treatment. Based on the investigation results, the author discussed the issues of triage of wound, informed consent, right of privacy, and the right to self-determination of wound soldiers. She summarized the definition of the concept, contrasted the principle and method of our military and foreign military, listed the relevant ethical disputes, discussed ethical problems and confusions, put forward the related thinking. As a result, she gave some advice to the principle of triage of wounded, and pointed out the necessity and the importance of paying attention to the wounded informed consent, privacy, the right to self-determination, and put forward her own proposals to solve the problem.The fourth part is the thinking and suggestion. The author put forward four proposals on military medical ethics research and development from the macroscopic angle. The first one is to adapt to the development needs, recognize the differences, objectively treat it, learn from each other, adhere to the fine traditions of our army, pay more attention to the military medical ethics issues. Second is to treat the military demand as the fundamental and the international law system as the core, combined with the practice to ensure the feasibility, strengthen the construction of military medical ethics regulations. Third is to strengthen the training of medical personnel, improve the wartime doctor-patient relationship, strengthen the fairness, listening, pay attention to postwar life quality of wound soldiers, strengthen the rear hospital construction, improve the wounded treatment, normative medical behavior, and fundamentally improve the ethics cognitive level. The fourth one is to strengthen personnel training, to promote the construction of the subject of military medical ethics.Finally, a general summary of this article points out the innovation points and shortcomings of this paper, and gives the suggestions for future research work.This study did the investigation on cognitive level of army and local health workers of medical ethics issues in wartime lifesaving work. Through the statistically test and analysis of the data, it displays the current actual situation of ethics cognition level of related groups and the degree of popularization and the feasibility of the present laws and regulations. This paper based on the real data, systematically summarized and comparative researched the principle in our and foreign army, ethical disputes and confusion on the key problems of medical ethics in wartime medical treatment work. It put forward its own opinion to the triage principle of wound in battlefield, and discussed the right of informed consent and privacy in-depth, which is a beneficial exploration on the research of relevant problems and the formulation of solutions. This has important reference value to promote the research of our military medical ethics and discipline, laws and regulations. It also provided a fundamental basis to the future development direction of military medical ethics research and the establishment and modification of the policies and regulations of wartime lifesaving work.
