The Improvement For The Disabled People’s Community Care Under Grid Management

For the disabled, the family and community is an important place for social life and communication. With the development of society, the demand of disabled is rapidly growth, and constantly extend to the social participation, cultural and entertainment, self-development and other high-level development. Due to the weakening of family care function, community care has gradually become the main model for the disabled. Disabled community care in China has long been, and play an important role in the field of care for the disabled people.Through the professional practice in the H street of Wuhan City,author integrated the grid management, community care, social support for the disabled and other related literature. Then used the grid management ideas and technology tools to improve the ocial support in the disabled community care field. Author based on the social support theory to explore the improvement of social support system for disabled people’s community care under the grid management,and using the grid management technology tools from the grid system, community system, peripheral system, professional organizations involved, information service etc. several aspects structure built the disabled community care of grid system, with the grid technology tools to integrate resourse of community care,improve the community care network of support.Through the questionnaire survey and in-depth interview methods to investigate the status and requirements of the 50 service objects in H street, and then summarize the main problems and requirements of sample groups. On this basis, using the grid system for the sample group of medical rehabilitation, daily life care, social interaction, psychological support and other services to intervene, and clear the principle and strategy of the intervention process.Finally, through the comparison with related care models, author tries to make an analysis on the grid system architecture of the disabled people’s community care.
