Objective: To evaluate of the left ventricular diastolic function in patients with hypertension by tissue Doppler imaging.Methods: 103 hypertensive patients were divided into 2 groups,which are hypertension without left ventricular hypertrophy(LVH)group and hypertension with LVH group.45 normal volunteers were employed as contrasrt group.Echocardiography were practiced in 3 groups,left atrial area(LAA),left ventricular ejection fraction(LVEF)were measured.Diastolic mitral flow E,A,E/A were measured by pulsed wave Doppler.Mitral annulus velocity e',a',e'/a' were assessed by tissue Doppler imaging.Results:1 Comparision of general clinical parameters between controls and patients with hypertensionAge,heart rate and BMI did not significantly differ between controls and hypertensive patients(P>0.05).Compared to controls,the hypertensive patients had higher systolic blood pressure(SBP)and diastolic blood pressure(DBP)(P<0.01),and SBP in group LVH was higher than that in group LVN(P<0.05).The group LVN exhibited greater LAA than controls(P<0.05),and LAA in group LVH was significantly greater than that in other 2 groups(P<0.01).Left ventricular myocardial mass(LVM)in hypertensive patients was weightier than that in controls(P<0.01),and LVM in group LVH was weightier than that in group LVN(P<0.05).Left ventricular systolic function did not significantly differ between controls and hypertensive patients(P>0.05).2 Comparison of left ventricular diastolic function parameters characteristics between healthy controls and patients with pulmonary hypertension(hypertension)Compared to controls,the spectral Doppler measured a decrease of mitral diastolic blood flow velocity E peak decreased(P<0.05),an increase of A peak(P<0.05),and a decrease of E/A in hypertension patients.These changes were more obvious in LVH group.Tissue Doppler measured a decrease of mitral annulus velocity(e')(P< 0.01)and a decrease of e'/a' ratio(P< 0.05)in hypertension patients,which was more evidently in LVH group.These variation indicated the left ventricular diastolic functions of the hypertension decreased in comparison with the normal control group.E/e'representativing left ventricular filling pressure was obviously higher than the control group.3 Correlations between left atrial area and left ventricular diastolic function parametersLAA significantly positive correlated with E/e',but showed no obvious correlation with E/A.Conclusion:1 e' and E/e' measured by tissue Doppler imaging can reflect sensi tively the damaged condition of left ventricular diastolic function in hyp ertension patients,which was more accurate than E peak and E/A meas ured by spectral Doppler imaging.2 e' and E/e' measured by tissue Doppler imaging significantly posi tive correlated with LAA,and could reflex sensitively left ventricular m yocardial remodelling.
