| Objective: In recent years,the incidence of thyroid carcinoma shows an increasing tendency worldwide.Though surgery and medication had good effects in most cases,there were 10%-20% cases of thyroid carcinoma still died of poorly differentiated and progressive carcinoma.Thyroid cancer can be classified by differentiated thyroid carcinoma and undifferentiated thyroid carcinoma according to the histological classification,and differentiated thyroid carcinoma can be classified by papillary thyroid carcinoma(PTC)and follicular thyroid carcinoma(FTC).Of all types of thyroid carcinoma,the former accounts for 75%,and the latter accounts for 16%.At present,there was still lack of tumor marker which can differentiate benign and malignant of the thyroid nodule and predict the prognosis effectively.The role of erythropoietin(EPO)in tumor genesis and development is becoming a research hotspot now,while there was no explicit conclusion of the EPO expression in thyroid carcinoma and its relationship with the biological characteristics of thyroid carcinoma.The purposes of the study was to explore the expression and role of EPO in differentiated thyroid carcinoma,and to find a specific and sensitive tumor marker for thyroid carcinoma which could be used for early diagnosis,put forward reasonable treatments and predict the prognosis.Methods: The thyroid resected tissues was selected from the complete data combined with the archive from the pathology department and the clinical data of the second hospital of Hebei medical university from 2008 to 2014.The diagnoses of all the specimen were confirmed after the reexamination by two senior pathologists.All the cases were primary cases which were not treated by radiotherapy,chemotherapy or endocrine therapy before surgeries,and there were no potential influence factors on the experimental result.This study included 40 cases of papillary thyroid carcinoma,20 cases of follicular thyroid carcinoma and 20 cases of nodular goiter which was selected randomly.All samples were tested by immunohistochemical-staining,and the immunohistochemistry scores(IHS)were estimated by the universal standard.SPSS 19.0 was used for statistical analysis.The statistical analysis was performed by SPSS 19.0,the measurement data were expressed as mean ± standard deviation,and compared by t test.P-value<0.05 was considered as statistical significant.Results: EPO was expressed in all thyroid tissues.The immunohistochemistry scores(IHS)of EPO were 4.65±2.54 in nodular goiter,5.88±1.98 in papillary thyroid carcinoma,and 7.70±2.18 in follicular thyroid carcinoma respectively,showing a rising tendency in three groups.There were significant statistical differences between nodular goiter group and follicular thyroid carcinoma group(P<0.001).The statistical differences between papillary and follicular thyroid carcinoma group were significant(P<0.002).The P-value between nodular goiter and papillary thyroid carcinoma group was 0.068.The IHS of thyroid carcinoma groups was higher than nodular goiter group(6.48±2.21 vs.4.65±2.54,P=0.003).Meanwhile,the IHS of follicular thyroid carcinoma group was higher than that of differentiated thyroid tissues(7.70±2.18 vs.5.47±2.24,P<0.001).Conclusion: EPO was highly expressed in differentiated thyroid carcinoma and related to tumor differentiation.EPO may play an important roles in genesis,development,invasion and metastasis of differentiated thyroid carcinoma.Testing EPO could have important clinical significance in diagnosis,typing and classification,treatment and prognosis prediction of thyroid carcinoma. |
