Objective: To explore the clinical significant of ultrasound elastography in patients with cervical intraepithelial neoplasia.The cutoff value of young-s modulus value were explored by ultrasound elasticity imaging techniques within normal cervix?cervical intraepithelial neoplasia and cervical cancer.Methods: One hundred and eighty-one participants were selec-ted from January 2015 to January 2016 in the Fourth hospital of Hebei Medical University.The subjects divided into cervical cancer 84 cases,cervical precancerous lesions 47 cases and normal cervix 50 cases.All parameters were examined by transvaginal ultrasound.The haemodynamics of the cervix was observed.The resistance index(RI)were measured and recorded.The Young's modulus value were measured by ultrasound elasticity.All parameters were analyzed by SPSS 13.0 statistical software.Two methods will be comparedwith the pathological results,build the receiver-operating curve(ROC),to find the cutoff value of cervical intraepithelial neoplasia and cervical cancer's young?s modulus value.Results:1 The resistance index of normal cervix were statistically difference between premenopausal and postmenopausal P<0.05.2 The premenopausal resistance index presented statistical difference among the normal cervix,cervical precancerous lesions and cervical cancer P< 0.05.Furthermore,The premenopausal resistance index were difference between cervical precancerous lesions and cervical cancer.3 The resistance index of cervical cancer presented statistical difference between premenopausal and postmenopausal P<0.05.4 Premenopausal resistance index's cutoff value of normal cervix and cervical cancer was 0.60.The sensitivity,specificity,PPV,NPV and accuracyof the elastosonography in normal cervix and cervical cancer were 81.7%,77.4%,87.5%,68.6% and 80.2% ? Premenopausal resistance index's cutoff value of cervical intraepithelial neoplasia and cervical cancer was 0.59.The sensitivity,specificity,PPV,NPV and accuracy of the elastosonography in cervical intraepithelial neoplasia and cervical cancer were 78.3%,76.9%,83.9%,69.8% and 77.8%?5 The hardness of the cervix among premenopausal,postmenopausal,Nulliparous and multipara's young?s modulus were not statistically difference(P>0.05).6 The difference between the normal cervix and cervical precancerous lesions with young?s modulus value presented no statistical significant(P>0.05).The difference between cervical cancer‘s young?s modulus value with cervical intraepithelial neoplasia and cervical cancer were difference(P <0.05.7 The difference between cervical cancer‘s young?s modulus value in the different cervical cancer type and the different stages presented no statistical significant(P>0.05).8 The calculated cutoff value was 5.09 with it sensitivity,specificity,PPV,NPV and accuracy of the elastosonography in normal cervix and cervical cancer were 92.9%?84.0%?90.7%,87.5%?89.6%,respectively.The calculated cutoff value was 5.05 with it sensitivity,specificity,PPV,NPV and accuracy of the elastosonography in cervical intraepithelial neoplasia and cervical cancer were95.2%?85.1%?92.0%,90.9%?91.6%,respectively.9 The accuracy were defference between the peripheral blood flow resistance index and cutoff value of Young's modulus value(P < 0.05.Obviously,the young's modulus was higher accuracy by ultrasound elasticity imaging than the peripheral blood flow resistance index.Conclusions: Ultrasound elasticity imaging techniques can be used in the differential diagnosis of cervical lesions and cervical intraepithelial neoplasia.The accuracy of Ultrasound elasticity imaging techniques is higher compared with color doppler ultrasound examination.Ultrasound elasticity imagingtechniques is a kind of differential diagnosis of cervical precancerous lesions and cervical cancer.But ultrasound elasticity imaging techniques can not be further identificated cervical cancer type,the different stages.
