Objectives:To analyze the prevalence and clinical features of type 2 diabetes mellitus(T2DM)with osteoporosis and osteopenia,explore the correlates of osteoporosis and osteopenia,and provide scientific evidences for preventing osteoporosis and osteopenia in patients with type 2 diabetes mellitus.Methods:A retrospective review was conducted in Metabolic Disease Hospital of Tianjin Medical University between 2011 August 30 and 2012 May 30,extracting relevant information of who were the first hospitalization.Patients with bone density scans were enrolled in this study,then we analyzed the prevalence and clinical features of type 2 diabetes mellitus with osteoporosis and osteopenia.A case-control study was used,the subjects with osteoporosis and osteopenia as case groups respectively and the subjects with normal bone as control group in type 2 diabetes mellitus inpatients.Data were analyzed using unconditional logistic regression and OR value and 95%CI were calculated.Results:1.The prevalence of type 2 diabetes mellitus with osteoporosis and osteopenia 2090 T2DM were enrolled in Metabolic Disease Hospital of Tianjin Medical University between 2011 August 30 and 2012 May 30,only 1353(64.7%)with bone density scans or diagnosed as osteoporosis and osteopenia were eligible for the study,including 469 osteoporosis and 501 osteopenia,the prevalence of type 2 diabetes mellitus with osteoporosis and osteopenia was 34.7%and 37.0%respectively,the prevalence of type 2 diabetes mellitus with abnormal bone(osteoporosis and osteopenia)was 71.7%;The prevalence of type 2 diabetes mellitus with osteopenia in male(40.0%)was higher than in female(33.9%),the prevalence of type 2 diabetes mellitus with osteoporosis in female(47.1%)was higher than in male(22.8%),the prevalence of type 2 diabetes mellitus with abnormal bone in female(81.1%)was higher than in male(62.8%);The prevalence of type 2 diabetes mellitus with osteopenia decreased with age,and yet,the prevalence of type 2 diabetes mellitus with osteoporosis increased with age and duration of diabetes,there is no noticeable change between duration of diabetes and the prevalence of type 2 diabetes mellitus with osteopenia.2.The clinical features of type 2 diabetes mellitus with osteoporosis and osteopenia Compared to the subjects with type 2 diabetes mellitus with noramal bone,the subjects with type 2 diabetes mellitus with osteoporosis and osteopenia had the following characteristics:higher age,higher age of diabetes diagnosis,longer duration of diabetes,and higher systolic blood pressure,higher rate of female,retirement,diabetic retinopathy and diabetic peripheral neuropathy,while lower rate of on the job,married,smoking,drinking and untreated,lower body mass index,lower triglyceride and lower blood uric acid;There were no statistical significance between the three groups of nation,diastolic blood pressure,family history of diabetes,use of insulin,diabetic nephropathy,dyslipidemia,hyperuricemia,hyperfibrinogenemia,fasting plasma glucose,hemoglobin Alc,total cholesterol,high-density lipoprotein,low density lipoprotein,blood calcium,blood phosphorus and C reactive protein(p>0.05).3.The correlates of osteoporosis and osteopenia In the univariate logistic regression analysis,adjusted by related factors,the results showed that female,age≥50 years old,unmarried,and duration of diabetes>7.0 years were associated with higher risk of type 2 diabetes mellitus with osteoporosis,while overweight and obesity were associated with lower risk of type 2 diabetes mellitus with osteoporosis;Female,age>50 years old,unmarried,minority nationalities,high alkaline phosphatase and high blood phosphorus were associated with higher risk of type 2 diabetes mellitus with osteopenia,while obesity was associated with lower risk of type 2 diabetes mellitus with osteopenia;There were no statistical significant associations between occupation,smoking,drinking,family history of diabetes,age of diabetes diagnosis,hypertension,diabetic retinopathy,diabetic nephropathy,diabetic peripheral neuropathy,fasting plasma glucose,hemoglobin Alc,triglyceride,total cholesterol,high-density lipoprotein,low density lipoprotein,blood uric acid,blood calcium,C reactive protein,fibrinogen and type 2 diabetes mellitus with osteoporosis and osteopenia(p>0.05).Significant factors in univariate logistic regression analysis were analyzed use Unconditional logistic regression,results showed that there were statistical significant associations between age,sex,duration of diabetes,overweight,obesity and type 2 diabetes mellitus with osteoporosis(p<0.05),age>50 years old,female-duration of diabetes>7.0 years were associated with higher risk of type 2 diabetes mellitus with osteoporosis,while overweight and obesity was associated with lower risk of type 2 diabetes mellitus with osteoporosis;There were statistical significant associations between age,sex,obesity and type 2 diabetes mellitus with osteopenia(p<0.05),age≥50 years old,female were associated with higher risk of type 2 diabetes mellitus with osteopenia,while obesity was associated with lower risk of type 2 diabetes mellitus with osteopenia.Conclusions:1.The prevalence of type 2 diabetes mellitus with osteoporosis and osteopenia was 34.7%and 37.0%respectively;The prevalence of type 2 diabetes mellitus with osteopenia was higher in male,while the prevalence of type 2 diabetes mellitus with osteoporosis was higher in female;The prevalence of type 2 diabetes mellitus with osteopenia decreased with age,and yet,the prevalence of type 2 diabetes mellitus with osteoporosis increased with age and duration of diabetes.2.Age≥50 years old,female,duration of diabetes>7.0 years were associated with higher risk of type 2 diabetes mellitus with osteoporosis,while overweight and obesity was associated with lower risk of type 2 diabetes mellitus with osteoporosis;Age≥50 years old,female were associated with higher risk of type 2 diabetes mellitus with osteopenia,while obesity was associated with lower risk of type 2 diabetes mellitus with osteopenia.
