?Objective? To investigate the correlation between blood glucose fluctuation and prognosis factors in patients with type 2-diabetes mellitus complicated with cerebral infarction, and to provide theoretical guidance for the treatment of acute cerebral infarction.?Methods?857 patients with cerebral infarction from June 2015 to December 2016 in the First Affiliated Hospital of Fujian Medical University, patients within 3 days of onset of cerebral infarction in patients with 421 cases, including 215 cases with type 2-diabetes,according to the exclusion criteria of removal in 19 cases,196 cases in group of cerebral infarction with type 2-diabetes,The general data were collected: gender, age, blood pressure, admission glycosylated hemoglobin, fasting blood glucose, blood glucose 2 hours after meal,fibrinogen,D-Dimer,homocysteine,total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein, past history of hypertension, history of coronary heart disease, cerebral infarction, smoking history,or the history of transient ischemic attack, factors of peripheral artery disease data,13 cases of incomplete data were excluded, and finally 183 patients into the group.collect peripheral blood sugar value after admission, calculate the mean, standard deviation, coefficient of variation of blood sugar.NIHSS score,mRs score,Barthel index score of all the patients were followed up 3 weeks later by face to face or telephone follow-up.Grouped according to the NIHSS score,mRs score,Barthel index score,Divided into neural function defect group(NIHSS score > 3 points),neural function defect group(NIHSS? 3 points);Have the basic life ability(Barthel index ? 60),disability group(Barthel index score < 60);The good prognosis group(mRs score < 2),poor prognosis group(mRs score = 2).Univariate analysis was carried out within the group, and the variables with statistical significance were analyzed by multivariate analysis.?Results?1.According to mRs score group within the group:Fibrinogen, total cholesterol, blood glucose standard deviation, blood glucose variation coefficient are statistically significant(P value were 0.049,0.028,0.02,0.1).blood glucose variation coefficient is a high risk factors of high mRs score(OR a value of 6.347),and P < 0.05,High blood cholesterol is the protection of the poor prognosis factors(OR value of 0.217),and P < 0.05.2.According to NIHSS score group within the group: History of heart disease, fibrinogen, total cholesterol, blood sugar variation coefficient is statistically significant(P P < 0.05.).High cholesterol is a protection factor for high NIHSS score 3 weeks after admission(OR a value of 0.684),and P < 0.05,High fibrinogen, high blood glucose variation coefficient are all risk factors(OR a value of 3.27,respectively,3.27),P < 0.05.3.According to the Barthel index score group within the group: History of cerebral infarction, fibrinogen, total cholesterol, low density lipoprotein, blood glucose standard deviation, blood glucose variation coefficient, glycosylated hemoglobin,D-dime has statistical significance(P < 0.05.).high D-dimer,high glycosylated hemoglobin, high blood glucose variation coefficient was risk factors of poor prognosis(OR a value of 4.957,4.957,7.126,respectively),and P < 0.05..?Conclusion?Acute stage of type 2-diabetes mellitus complicated with cerebral infarction, the high glycosylated hemoglobin, high blood glucose variation coefficient, blood glucose standard deviation, the poor prognosis, severe neurological deficit, poor self-care ability.
