Objective: To summarize occurrence regularity and influence factors of the different age groups of inpatients with herpes zoster,explore ways for early diagnosis of herpes zoster,resons of misdiagnosis and treatment methods,and speculate factors which are associated with herpes zoster.To provide theoretical basis for clinical diagnosis and treatment of the disease and find optimal treatment protocols.Methods: 208 cases from inpatients of herpes zoster in our department in recent three years were selected by random number.According to age,the patients chosen were divided into 2 groups,including 119 patients aged less than 60 and 89 patie nts aged over 60 years.Moreover,patients were also grouped by the use of sugar cortical hormone,analgesics,antibiotics and so on.The clinical datas were collected and differences were compared between groups,using SPSS predictive analytics so ftware.Results: The sex ratio was 1:1.04 of 208 patients,and the ages were 9 to 85 years old,with peak incidence of ages range from 50 to 59 years.The number of patients of 4 seasons accounted for 27.40%,27.40%,21.63% and 23.56%,respectively.The common underlying diseases of the patients were hypertension,type 2 diabetes,cancers and so on.And the common precipitating factors of herpes zoster were fatigue and upper respiratory tract infection.Systemic symptoms appeared in 6.73% patients,99.04% patients suffered this disease for the first time,47.60% patients felt pain before the find of rashes.26 patients were misdiagnosed,most of whom had prodromal pain.Herpes zoster commonly appeared on head and face,neck and shoulder,truncus.The affected nerve mainly included intercostal nerve,trigeminal nerve and cervical nerve.62 patients’ vesicular fluid were sent to the laboratory for bacterial culture,which was positive in 15 cases,and only 1 case of drug sensitivity suggested multi-drug resistance.There was statistically significant difference in the incidence rate of underlying diseases,hospitalization days,initial symptom,course of antiviral drugs and antibiotics,glucocorticoid usage rate,pain degree on admission and discharge and clinical path entrance rate between group A and group B(P<0.05);However,pathogenic sites,nerves involved,time from symptom onset to the initiation of treatment,kinds of antiviral drugs,usage rate of analgesic and antibiotics,prognosis of the disease and rate of complications between different ages of patients had no statistical significant difference(P>0.05).Patient outcomes included cure(159 cases),improvement(47 cases)and uncure(2 cases).The incidence of loss to follow-up after discharging was 1.44%.Our analysis of 205 cases showed PHN was more likely to appeare in elderly patients,those whose initial treatment time were more than 3 days and patients didn’t use glucocorticoids(P<0.05),while gender and prodromal pain had no significant correlation with PHN(P>0.05).The hospitalization expenses increased with the increase of hospitalization.Linear correlation coefficient was more than 0.7,indicating that they were highly correlated.The total clinical path entrance rate was 22.12%,and was even lower among elderly patients.Conclusion:(1)The incidence of herpes zoster has no obvious relationships with gender and season,but is positively correlated with age.(2)The elderly,with higher incidence of underlying diseases and severer pain,have longer stay in hospital and longer course of anti-viral drugs and analgesic.(3)The incidence rate of PHN is relevant with age,time from symptom onset to the initiation of treatment,glucocorticoid usage,but not gender and precursory pain.
