Objective1.To summarize the clinical data of patients with herpes zoster and analyze the characteristics of herpes zoster.2.To explore the influencing factors of postherpetic neuralgia and to provide reference for clinical diagnosis and treatment.Methods This retrospective study analyzed 123 cases of herpes zoster hospitalized in Department of Dermatology,General Hospital of Tianjin Medical University from July 2017 to December 2017.1.Recorded conditions of patients' gender,age,length of stay,predisposing factors,chronic basic diseases,first symptoms,initial diagnosis,involved nerve,clinical classification,pain degree and other clinical data.2.To explore the influencing factors of postherpetic neuralgia by univariate analysis and multivariate analysis.Results1.Among the 123 HZ patients,there were slightly more females than males,and ratio of males to females was 0.89:1.The average age of onset was 61.3 years old,the youngest age was 23 years,the oldest age was 92 years old,and the proportion of patients over 50 years old accounted for 80.5%.The total hospitalization days were1244 days,and the average hospitalization days were 10.11 days.The subjects were divided into five groups according to their age: < 50,50-59,60-69,70-79,? 80,no significant difference in hospitalization days between different age groups(P >0.05).There were 31.6% of the patients with definite predisposing factors.Patients with chronic basic diseases accounted for 71.5%,and 12 patients had malignant tumors,most of which were lymphoma.The first symptom was 50.5% of the patients with pain.In the case of non-dermatology in the first diagnosis department,the patients with the first symptom of pain were significantly more than those with the rash and pain(P<0.008).The nervi cerebrales was the most frequently affected.The lesions were slightly more on the left side than on the right side,and the ratio between the left and the right side was 1.18: 1.The most common type in the general clinical classification was the common type.Accounting for 87.8%.The most common in special clinical classification is trigeminal herpes zoster.Some of the subjects can be combined with auricular herpes zoster at the same time.The pain degree was mainly3 and 4,accounting for 96%,and there was no significant difference in the pain degree of the same age group(P > 0.05).There was no significant difference in the time of decrustation and pain relief in different age groups(P > 0.05).2.Univariate statistical results of the influencing factors of postherpetic neuralgia showed that age,skin lesion distribution,clinical type,gender,glucocorticoid use were not statistically significant,and the interval between onset and antiviral therapy was not significant.Multivariate Logistic regression analysis showed: age,skin lesion distribution,clinical classification,gender,glucocorticoid use,interval between onset and antiviral therapy,involved nerves were not statistically significant,and there was significant difference in pain degree.The pain of acute-stage herpes zoster was the risk factor of postherpetic neuralgia(P < 0.05 OR > 1).Conclusions1.The incidence of herpes zoster increases with age.2.Women have a higher incidence than men.3.Patients with unexplained unilateral skin pain should be alert to the possibility of herpes zoster in the clinical department of non-dermatology department.4.Herpes zoster patients with initial symptoms of pain are easily misdiagnosed.4.Among the hospitalized patients with herpes zoster,the cerebral nerve was the most involved,followed by the thoracic nerve and lumbosacral nerve.5.PHN is the most common sequelae of patients with herpes zoster.6.The degree of acute herpes zoster pain can be regarded as a risk factor of postherpes neuralgia.
