Risk Factors And Ultrasonic Features Of Nodular Goiter With Thyroid Carcinoma

Objective:Through the case-control study on the patients with nodular goiter with thyroid carcinoma,we investigate risk factors and ultrasonic features of nodular goiter with thyroid carcinoma,in order to improve diagnostic accurate rate before surgery.Methods:183 pathologically diagnosed cases of with nodular goiter admitted to the general surgery of the Second Affiliated Hospital of Dalian Medical University from January 2013 to January 2014 are retrospectively analyzed,whose clinical data is complete.These cases include 77 patients with nodular goiter with thyroid carcinoma and 106 patients with nodular goiter,as case group and control group respectively.The patient's age,sex,blood TSH level,level of autoantibodies,body mass index,history of radiation exposure,family history,history of pregnancy are studied as risk factors,and the number of nodular,calcification,echo type of nodular,neck lymph node,spicule sign are studied as ultrasonic features.The measurement data is analyzed by two independent sample t-test,the enumeration data is analyzed by chi-square test for categorical data,the ranked data is analyzed by rank sum test.The risk factors and ultrasonic features that are significantly different are filtered out and once again analyzed by multivariate logistic regression analysis.Finally,the risk factors and ultrasonic features with nodular goiter with thyroid carcinoma are ensured.Results:In the single factor analysis,the level of autoantibodies(TGAb,TPOAb),history of radiation exposure,family history,calcification,microcalcification,echo type of nodular,spicule sign have the differences of statistics(P<0.05).Through multivariate logistic regression analysis,we finally determine the level of TPOAb,history of radiation exposure,family history as the risk factors and microcalcification,hypoechoic nodular,spicule sign as the ultrasonic features with nodular goiter with thyroid carcinoma.Conclusion:The risk factors of nodular goiter with thyroid carcinoma include the level of TPOAb,history of radiation exposure,family history.The ultrasonic features with nodular goiter with thyroid carcinoma include microcalcification,hypoechoic nodular,spicule sign.
