Risk Factors Of Nodular Goiter Combined With Thyroid Cancer

Objective:To explore the clinical and imaging features of nodular goiter and thyroid carcinoma,to find the risk factors of nodular goiter and thyroid carcinoma,and to provide the basis for the diagnosis and treatment of nodular goiter and thyroid carcinoma.Methods:The clinical data of 538 patients with nodular goiter with surgical indications from December 2014 to December 2017 were collected.Among the 538 patients,496 cases were nodular goiter,92,1%(496/538),nodular adenoma and thyroid carcinoma in 42 cases,accou-nting for 7.8%(42/538).Through the analysis of clinical data,we selected the number,size,calcification,echo,internal structure,cervical lymph node enlargement and 8 factors of the individual's sex and age as a single factor,and the multiple regression analysis was conducted on the basis of single factor analysis.Nodular goiter is an independent risk factor for thyroid cancer.The clinical data of 42 patients with nodular goiter complicated with thyroid cancer were summarized.Results:A total of 538 samples,406 cases of bilateral thyroid nodules,75.4% of the thyroid nodules,132 cases in one side,24.5%,88 of single nodules,16.3%,45 of multiple nodules,83.7%,149 nodules,27.6%,no calcification,accounted for 72.5%;24 cases of echo nodules,accounted for 4.5%,238 cases of hypoechoic nodules,44.3%,18 cases of equal or hyperech-oic nodules,18,3.3,258 mixed echo nodules,47.9%,24 cystic nodules,4.5%,256,occupied specific proportion,47.6%;cystic nodule 258 cases,accounted for The weight is 47.9%.In the single factor analysis of nodular goiter and thyroid carcinoma,there were no significant differ-ences in 5 factors(P>0.05)for the sex,age,echo of nodules,internal structure of nodules,and cervical lymph node enlargement.The number of thyroid nodules,the size of nodules,and calcification were statistically significant(P<0.05)(P<0.05),that is,the above 3 factors were associated with nodular goiter and thyroid carcinoma.Multiple Logistic regression analysis of nodular number,diameter and calcification showed that the 3 factors were negatively correla-ted with the risk of nodular goiter with thyroid cancer,and nodular calcification was positively related to nodular goiter and thyroid cancer.Conclusion:1.The risk factors for the development of nodular goitre and thyroid cancer are the number of nodules,the size of nodules,and nodule calcification(especially microcalcification).2.Nodular calcification(especially microcalcification)increases the risk of nodular goiter and thyroid cancer,and positive surgical treatment should be performed on nodular goiter calcification,especially gravel-like calcification.
