| Objective:By observing the ginseng and astragalus righting injection in patients with primary nephrotic syndrome(PNS)after application of glucocorticoid treatment the influence of T lymphocyte subsets,to explore traditional Chinese medicine to improve PNS patients after long-term use of glucocorticoid for adjustment disorder possible mechanisms of the body's immune function.Methods:1.According to the study of the inclusion criteria,the selected 62 patients were randomly divided into control group and treatment group,control group 22 cases,40 cases in treatment group.Control group is given priority to with sugar cortical hormone standard therapy and general foundation treatment.Treatment group in the control group of conventional western medicine therapy on the basis of factors to ginseng and astragalus injection 250 ml,intravenous drip,1 times a day,even a dosing,observation period of 14 days.2.Observe the indicators are:(1): laboratory test index before and after treatment in the two groups patients,24 hours urinary protein,renal function,plasma albumin,quantitative T lymphocyte subgroup: CD3+,CD4+,CD8 + and the ratio of CD4+/CD8 + and NK cell level;(2)the security indexes: blood routine(RBC,WBC,Hb,PLT),check liver function(ALT,AST),electrocardiogram(ecg).3.SPSS19.0 Statistical processing: using statistical software for data analysis,measurement data with mean +/-standard deviation(x ąs)said,count data by chi-square test and expressed as a percentage,grade adopts Ridit test data measurement test,the comparison between groups using t test,with P<0.05 for the difference is statistically significant.Results:1.Overal syndrome curative effect evaluation: 1.Two groups of patients after the whole course of treatment,the treatment group syndrome curative effect of the total effective rate was 95.0%,the control group was72.7%,two groups total effective rate have significant difference(P<0.05).Show that syndromes curative effect of treatment group was obviously better than the control group.2.The TCM symptom score evaluation: two groups after treatment in patients with significant difference between the total score of TCM symptoms(P<0.05),indicating that the treatment group is superior to control group on the improvement of the patients with symptoms of TCM.3.A single symptom integral evaluation: before and after treatment,the treatment group the symptom integral have significant difference(P<0.05),the treatment group after treatment than before treatment;The control group only body stay trapped weight,eat less,these two points there is a significant difference before and after the treatment(P<0.05),the scores of other no statistically significant difference before and after treatment(P>0.05);And two groups of patients after treatment,the treatment group in the food,stay trapped,shortness of breath lazy speech,body weight,fatigue,spontaneous perspiration susceptible symptoms improved better than that in control group(P<0.05).4.Two groups of patients before and after the treatment,24 hours urinary protein quantitative: before and after treatment between two groups of patients within the group,group of 24 h urine protein quantitative statistical results show that the differences between the two way inside,group had no statistical significance(P>0.05).5.Compare two groups of patients before and after treatment plasma albumin levels,before and after treatment between two groups of patients within the group,group statistical results show that the plasma albumin levels within two way,differences between groups were no statistical significance(P>0.05).6.Compare two groups of renal function indexes before and after treatment,two groups of patients before and after the treatment group,group,renal function indexes between the statistical results show that the differences between the two way inside,group had no statistical significance(P>0.05).7.The two groups before and after treatment comparison: T lymphocyte subsets index before and after treatment,the treatment group on the level of T lymphocyte subsets in patients with(CD3+,CD4+,CD8+)and CD4+/CD8+ratio,level of NK cells was increased,the difference is statistically significant(P<0.05);And the control group was reduced,the but no statistically significant difference(P>0.05);Before and after treatment,the treatment group CD8+level has decreased compared with before treatment,the difference is statistically significant(P<0.05);While the control group of CD8+levels before the treatment was increased,but no statistically significant difference(P>0.05);Specification,before and after treatment,the treatment group can obviously improve patients T lymphocyte subsets(CD3+,CD4+,CD8+),CD4+/CD8+ ratio,level of NK cells.8.Safety index evaluation: at the completion of 62 cases of patients with disease observation in the process of the 24 h urine protein in patients with quantitative,plasma albumin levels and renal function in a stable state,and the patient's blood routine,liver function,electrocardiogram(ecg)were found no clinical significance of abnormal index,adverse reactions,has not been found that ginseng and astragalus righting injection to improve the patients with primary nephrotic syndrome caused by long-term glucocorticoid treatment level of T lymphocyte subsets changes treatment is safe.Conclusion:This study by observing the ginseng and astragalus righting injection for patients with primary nephrotic syndrome(PNS)after long-term use of glucocorticoid for the influence of the level of T lymphocyte subsets,explore the drug improve long-term daily use of corticosteroids in patients with PNS,possible mechanisms of immune dysfunction after treatment.Studies show that ginseng and astragalus righting injection can increase CD3+,CD4+,CD4+/CD8+and NK cell level,reduce the level of CD8+,factors that ginseng and astragalus injection can adjust the level of T lymphocyte subsets to improve PNS after long-term use of glucocorticoid for the body's immune function disorder,and clinical curative effect,worth clinical promotion. |
