Vertebroplasty Combined With Invigorating Kidney And Activating Blood Chinese Medicine In The Treatment Of Osteoporotic Vertebral Compression Fractures

Objective:Retrospectively analysis the clinical curative effect of using invigorating kidney and activating blood Chinese medicine Combined with PKP in the treatment of Osteoporotic Vertebral Compression Fractures.Methods:The 86 patients' data with OVCFs(Osteopomtic Vertebral Compression Fractures)we collected were from the garden mountain district of Hubei Provincial Hospital of TCM from March 2014 to September 2015.Those Patients aged between 53 and 76 years old,average of 63.4 years old.All patients were undergone the surgery of PKP(Percutaneous Pyphoplasty), and divided into therapy group and control group according to whether accepting Chinese medicine.By the time they admitted to hospital,they all had back pain,and difficult to stand,walk and turn over in bed,there spines had different degree of deformation.When asking their medical history,they all felt back pain bone pain,and the pain raised or cannot move when bearing,difficult to walk,turn over and sit while seriously.There are 27 patients who had history of fractures in the other areas of body.Before the treatment, all patients underwent lumbar lateral DR,CT+3D reconstruction and bone densitometry( Bone Mineral Density, BMD).According to statistic analysis,the general data like sex and age between therapy group and control group before treatment were not significantly different,they are comparable.The diagnostic criteria of western medicine were according to the diagnosis standard of Primary osteoporosis recommended by the WHO in 1994[1]and the diagnosis standard of VCFs in Surgery(the eighth edition).The diagnostic criteria of TCM were according to the criteria of ganshenbuzu syndrome of osteoporosis in the guiding principles of the clinical research of new medicine of TCM[2]and the diagnostic criteria of VCFs in osteology and traumatology of traditional Chinese medicine.After the surgery,patients in TG were taken by invigorating kidney and activating blood Chinese medicine,patients in CG were taken by Oral calcitriol soft capsule and D-cal chewable tablet,the period of treatment were all 4 weeks.Before and after the treatment,using the visual analogue scale to assess the patients,pain,using the Oswestry disability index[3]to evaluate the improvement of daily activities,density the bone densitometry of hip to evaluate the shift,and evaluate the curative effects according to the standard of guwei in the industry standard of TCM in the People's Republic of China-the criteria of diagnose and curative effects of Chinese medicine. All the data in this observation were analysed by the software of SPSS 19.0.Results:86 patients in this observation achieved complete follow-up for6 ~ 10 months.The VAS scores were no difference between two groups(P>0.05)before and after treatment,they were comparable;Of which 1 day after the operation, the VAS scores of two groups were lower than before,the difference was statistically significant(P<0.05),and there was no significant difference between the two groups(P>0.05),they were comparable.The VAS scores after 1week and 1 month were reduced,compared with preoperative the values were statistically difference(P<0.05),and the scores differences between the two groups, the curative effect of TG was better than CG;There were no different between the two groups after 3?6 months on VAS(P>0.05).The two groups' ODI index were Obvious different before and after accepting treatment(P<0.01), statistically significant;Statistically analysis the ODI index of the two groups after received appropriate treatment through 1 week and 1month,the difference were statistically significant(P<0.05);And there were no different between the two groups after 3?6 months on ODI(P>0.05),the difference was not statistically significant(P>0.05).The BMD of the two groups of patients before the surgery were not significantly different(P>0.05),they were comparable;There were different after the two groups received treatment through 1 month on BMD(P<0.05),but it had no significantly significant between the two groups(P>0.05);Compared with the time before the surgery and the prior time,the patients' T score of BMD after received treatment 3?6month were different(P<0.05),and there were different between the two groups.After the treatment, the evaluation data and curative effect of the diseases and syndromes on TCM were statistical analysed by using ?2test,the different was statistically significant(P<0.01).Conclusion:The surgery of PKP can effectively reduce the back pain in the short time,regain the height of the injured vertebral body to a certain extent,and improve the quality of life of patients.But it will left back pain after the surgery,using invigorating kidney and activating blood Chinese medicine after the PKP in the Treatment of OVCFs can effectively relieve the pain and improve the function of patients in the short-term,in the medium and long term,it can enhance the BMD of body,the curative effect to OP of the patients themselves was distinct,the effect were better than western medicine.This treatment were simple,convenient,effective and cheap in clinical,have relatively minor toxic side effect,and it,s worthy of further clinical validation and promotion.
