OBJECTIVEBy analyzing Osteoporotic vertebral compression fracture(OVCF)and Osteoporotic vertebral burst fracture(OVBF)by the treatment of percutaneous kyphoplasty(PKP)imaging data,and investigate the leakage of bone cement leakage location,route,type of fracture on the clinical efficacy.METHODSSecond Affiliated Hospital of Nanchang University,January 2013-January2016 percutaneous kyphoplasty treatment of osteoporotic vertebral fractures 215 cases with 245 vertebrae.Postoperative patients were divided according to radiographic osteoporotic compression fractures and osteoporosis group burst fracture group.Met the inclusion criteria OVBF 35 vertebrae,OVCF there are 210.The bone cement leakage into the spinal canal,intervertebral space,and other parts of the leakage(paraspinal,paravertebral veins,needle tract).Analysis using VAS score in each group of patients with low back pain relief.RESULTSThe cement leakage group of OVBF and OVCF were statistically significant(P<0.05),but no significant difference between the two groups within the spinal canal bone cement leakage(P> 0.05).OVBF group intervertebral space and paraspinal soft tissue leakage rate is significantly higher than OVCF group,two of the sites were significantly different(P <0.05).Other parts of the leakage between the two groups was not statistically significant(P> 0.05).Vertebral bone cement by intravenous group,the posterior wall of the vertebral body,the inner wall of three ways pedicle broken into the spinal canal,but the vertebral vein group may be the main way to cement leakage within the spinal canal.OVBF group and OVCF by PKP group and last follow-up after treatment VAS score was not statistically significant(P <0.05).There are two groups of patients with and without bone cement leakage of bone cement before surgery postoperative VAS leaks are relatively undifferentiated(P<0.05).Two groups of patients in the bone cement leakage into the spinal canal,and after the end of the follow-up VAS scores were not statistically different(P> 0.05).Two groups of patients before bone cement leakage into the intervertebral space intraoperative and postoperative VAS score was not statistically significant(P> 0.05),but the preoperative and last follow-up VAS scores were significantly different(P<0.05).CONCLUTION(1)Percutaneous kyphoplasty treatment of osteoporotic vertebral fracture and osteoporosis fractures spinal compression fracture of bone cement leakage rate difference.Bone cement may leak into the spinal canal through the main vein vertebral group.(2)Vertebral osteoporotic fractures are more likely to rupture cement leakage,leakage and intervertebral paravertebral leakage rate was significantly higher than osteoporotic compression fractures.(3)Percutaneous kyphoplasty treatment of osteoporotic vertebral fractures of the spine and lower back pain relief has occurred independent of bone cement leakage,leakage location and type of fracture.But within the intervertebral space cement leakage may affect the patient's low back pain in a short time.
