ObjectiveTo explore the handgrip exercise regimen which can prevention the PICC-related upper extremity venous thrombosis from the perspective of clinical. Make exercise applications of electronic grips according to the strength frequencies and time period by this study designed and the changes of venous blood flow velocity and blood vessel diameter of upper extremity venous with PICC was measured by pulsed Doppler ultrasound to provide the basis of effective choice about the regimen of handgrip exercise for patients with PICC.MethodsThe first part:through literature retrieval and to consult, we find that the strength, frequency and time period of handgrip exercise in patients with PICC have no reference standard, so first choose appropriate age groups of healthy subjects use CAMRY electronic grip to look for the effective handgrip exercise regimen under the condition of color Doppler ultrasound testing the changes of venous blood flow velocity and blood vessel diameter. Select 300 healthy subjects whom years between 45to 65, divided into three phases A, B, C three groups to carry out, each group 100subjects and the ratio of male to female is 1:1, group A determined the appropriate strength; group B handgrip exercise by using electronic handgrip indifferent frequency (15 times/min,20 times/min,25 times/min,30 times/min) to determine the best handgrip exercise frequency; group C handgrip exercise in the most appropriate tap position with the best frequency and different time period (1min,2min,3min) to determine the feasible handgrip exercise time period.The second part:according to the above determine the handgrip exercise strength, frequency and time period set several different pattern to quantity daily exercise, on the basis of the changes of venous blood flow velocity and venous diameter in basilica vein with PICC to explore the best handgrip exercise pattern.ResultsThe first part:the healthy subjects which the age was 45?65, completed the handgrip exercise with the maximum grip strength and the best tap position was weak. Venous blood flow velocity and blood flow increased significantly compared with the corresponding values at resting state (p<0.05). When the tap position was weak, the frequency was 25 times/min, the time period was 2mins, the venous blood flow velocity and blood flow increased to the maximum (p<0.05). The sex factors that affect the results has no obvious difference.The second part:The venous blood flow velocity before and after PICC placement have significant differences (F=2.934, P=0.019<0.05) at different times. It were statistically significant (F=3.432. P=0.047<0.05)at venous blood flow velocity among three groups. The blood vessel diameter before and after PICC placement have significant differences (F=3.940, P=0.037<0.05) at different times. The blood vessel diameter before and after PICC placement was significant differences (t=2.113, P=0.039<0.05).ConclusionHandgrip exercise can promote blood circulation of upper limb, the best handgrip exercise mode by using electronic hand grip in healthy subjects is weak tap position, 25times/min and 2 mines time period; in patients with PICC using the electronic hand grip in weak tap position,25hold/min,2min every time, and 6 times a day (once every 3 hours), can obviously promote the circulation of the blood.
