The Relationship Between Traditional Chinese Medicine Constitution And Insulin Resistance Index And Gestational Diabetes Mellitus In Early Pregnancy

Objective: This thesis aims at discussing the relevance between traditional Chinese medicine physique type and IRI(Insulin Resistance Index)and GDM(Gestational Diabetes Mellitus),found the physical characteristics and the level of GDM in pregnant,providing a certain reference and theoretical basis for the early prediction of clinical GDM and the prevention of GDM through physical intervention to develop the "cure not ill" concept in traditional Chinese medicine.Methods: This topic adopts cross-sectional survey research methods.A sample survey was conducted among the pregnant women who regularly received health inspection in The First Affiliated Hospital of Henan Traditional Chinese Medicine University April 2015 to February 2016.A total amount of 230 effective questionnaires were taken back except for those whose information is not complete.105 cases of GDM pregnant women who conform to the diagnostic criteria were selected as case group,and the simple random method was used in selecting 105 cases from 125 cases of healthy pregnant women in the same period as control group.GDM epidemiological survey(see appendix 1)was used in the survey,the results was entered into SPSS and establish the database.The chi-square test was used to analyze the count data of two groups,and t test was used to analyze the measurement data;Fisher's exact probability method was adopted in comparing two groups of constitutional type composition analysis,two independent sample nonparametric test is used for statistical analysis.Results:(1)the GDM pregnant women shares something in common with normal pregnant women group: both shares gentle quality as the main constitution;in normal pregnant woman group,peace constitution constitute the main body constitution,gentle quality accounted for 61.9% of the total number of normal pregnant women,which explains peaceful mass is the main constitution in healthy women;(2)obvious differences exist in GDM pregnant women and normal pregnant women group,statistical analysis suggests differences statistically significant,the main biased constitution of TCM(traditional Chinese medicine)constitution identification in GDM group is phlegmatic hygrosis while normal pregnant women group is the yang deficiency;(3)case group and control group in insulin resistance index were statistically and significantly different(P < 0.05);(4)the insulin resistance index(IRI)is highest in the GDM phlegmatic hygrosis,compared with other constitutions,the difference is of statistically significance(P < 0.05);(5)Multi-factor Logistic regression analysis that these 4 dangerous factors are categorized as regression model,these 4 dangerous factors are weight before pregnant,diabetes history of family,phlegmatic hygrosis constitution and IRI.Regression coefficient individually are0.168,1.207,1.449,1.267.Conclusion:(1)phlegmatic hygrosis constitution is GDM susceptible biased constitution;(2)insulin resistance index is positively correlation with GDM and can early predict GDM;(3)Logistic regression result reveals that weight before pregnant,diabetes history of family and phlegmatic hygrosis constitution are 4 independent dangerous factors of GDM.
