The Application Of Dorsal Fascial Pedicle Flaps Repairing Skin And Soft Tissue Defects Of The Finger

Background:Hand trauma is common in clinical work. Skin and soft tissue defects of the finger is a common emergency in hand trauma, especially the lack of skin soft tissue through your fingers. The reasons of trauma and disease contain :crushed and destructive injury, incise injury, cutaneous necrosis caused by bruised and buring,section,necrosis after replantation.It is determined by age, job demand, aesthetic and functional requirement of fingers of patients.For example, right index finger was injuried,the requirement of repairing is higher comprised to the little finger injuried,we should retain index finger's length and function owing to it's important function.The requirements of the younger age tend to higher.The operation methods of wound regeneration,retaining finger's length and function include: debridement and suturing,free skin grafting, replantation in-situ of amputated tissue, cross finger skin flap,thenar flap, V-Y advanced flap, rectangle advanced flap, reversed digital artery flap,abdominal pedicle flap, free tissue transfer, toe-hand transfer and so on. The appearance and function of free tissue and toe-hand transfer is excellent,but it is hard to expanding dure to high risk and surgical technic.Advanced flaps can cure little skin and soft tissue defects except large area.Distant pedicle flap needs pedicle division in second phase, fingers with forced attitude and bloated appearance.The same finger skin flap can be completed in initial period and with attractive appearance.Reversed digital artery flap can achieve this aim,but it is shot of one side of digital artery. Dorsal fascial pedicle flaps not only achieve this aim,but protect digital artery.It is a simple and practicable surgery method which has been discussed.It has not been widely popularized due to various reasons?Objective:To analysis the anatomy of dorsal fascial pedicle flaps,the method of designing and harvestting flaps,caring and treatment in the postoperation, the revision surgery and the clinical data to form theory system of reversed dorsal fascial pedicle flaps which can be widely popularized.Methods :Repairing skin and soft tissue defects of the finger by reversed dorsal fascial pedicle flaps in the same finger with rotational part leave triangle pedicle.Taking same caring and treatment in the post-operation.Results :There are 26 clinical cases adopt the dorsal fascial pedicle flaps to repaireskin and soft tissue defects of the finger.One of them is failure owing to the defects of design or operation,the rest of them were alive with beautiful appearance and well function.Donor site is with less scar.Conclusion :The advantage of using reversed dorsal fascial pedicle flaps to repair skin and soft tissue defects of the finger are simple operation,high success rate,beautiful appearance,without damaging digital artery,less suffering,short hospital stay and less expenses. It is ideal flaps to repair skin and soft tissue defects of the finger and is worthy widely application in primary hospital.
