Objective1.To investigate the correlation of the ABO blood group with the clinicopathological characteristics in patients with gastric cancer and to assess whether the ABO blood group was associated with prognosis.2.To evaluate if there was a correlation between ABO blood groups and the survival of gastric cancer patients who underwent radical gastrectomy stratified by the preopera tive serum tumor markers in a C hinese population.3.The aim of this study was to investigate that if the predictive power of serum CEA is effective in four different ABO blood types. Methods1.Clinical and follow-up data of 2838 patients with gastric cancer who underwent radical gastrectomy in our department from June 2008 to October 2013 were analysed retrospectively.2.A total of 3307 gastric cancer patients who underwent radical gastrectomy were retrospectively analyzed at a single institution from Septe mber 2008 to April 2015. The Kaplan-Meier method was used to estimate the 5-year overall survival(OS). The baseline of patients with preoperative serum CEA-positive was determined by Chi-squared tests. The COX Proportional hazards model was applicated in univariate and multivariate analyses.3.Based on the previous study, A total of 3297 gastric cancer patients who underwent radical gastrectomy were selected and analyzed retrospectively at a single institution from September 2008 to April 2015. The Kaplan-Meier method was used to estimate the 5- year overall survival(OS), The COX proportional hazards model was applicated in univariate and multivariate analyses. Results1.There were no significant differences in the clinicopathological characteristics among patients with different ABO blood group; The 5-year overall survival(OS) rates were 57.3% for patients with blood type A, 54.7% for type B, 57.4% for type O, 53.5% for type AB. Though there was no significance difference between ABO blood groups and survival of the patients(P=0.722), in the subgroup analysis of patients withstage?, patients with blood group Non-AB had a poorer OS compared with blood group AB(P=0.014); in the subgroup analysis of patients who smoked, patients with blood group Non-AB had a Poorer OS compared with blood group AB(P=0.044).2.There was no statistically significant difference between ABO blood groups and the outcome of patients stratified by the four tumor markers. In a subgroup analysis of 660 patients with preoperative serum CEA-positive, patients with blood type AB had a better survival time than patients with blood type non-AB(P=0.020). The meidian OS time for these patients with the AB blood type and those with the non-AB blood type were 58.2 months and 25.2 months, with the 5-year OS rates of 45.8% and 35.4%, respectively. Multivariate analysis showed that blood type AB was a favorable and independent prognostic factor for gastric cancer patients with preoperative serum CEA-positive.3.Multivariate analysis showed that preoperative serum CEA was a unfavorable and independent prognostic factor for GC patients, additionally, for GC patients with blood type A, B, or O, CEA was a favorable and independent prognostic factor(all P<0.001), however, patients with blood type AB was not(P=0.956).The 5-year survival rates for patients with blood type A grouped by serum CEA-negative and CEA-positive were 61.7% and 33.5%(P<0.001), respectively; for type B were 61.3% and 38.9%(P<0.001), respectively; for type O were 62.4% and 32.2%(P<0.001), respectively; for type AB were 57.9% and 45.8%(P=0.189), respectively. Conclusions1.Neither clinicopathological characteristics nor overall survival were associated with the ABO blood group, however, in the subgroup analysis,Among patients with stage? and patients who smoked, patients with blood group Non-AB had a poorer OS compared with blood group AB.2.Blood type AB was a favorable prognostic factor for gastric cancer patients with preoperative serum CEA-positive.3.For patients with non-AB blood type, CEA was a unfavorable and independent prognostic factor. the preoperative serum CEA lost its predictive power in gastric cancer patients with the AB blood group.
