The Exploratory Study Of Individualized Tumor Vaccine Prepared With Pemetrexed Combined With DC-CIK On Advanced EGFR Wild-type Adenocarcinoma Lung Cancer

Objective:To explore the feasibility of individualized tumor vaccine prepared with Pemetrexed combined with DC-CIK on advanced EGFR wild-type Lung adenocarcinoma in vivo. Methods:One patient who suffered from adenocarcinoma lung cancer with brain metastases,EGFR-TKI mutation-negative and EML4-ALK-negative.The patient receive treatment in People's Liberation Army Four Severn Six Hospital.The patient received standard first-line chemotherapy pemetrexed combining cisplatin simultaneous whole brain radiation therapy to induce exposure of tumor antigen.Then,we reinfusion DC-CIK which was separated from the blood before chemotherapy,GM-CSF was also infused.The clincial efficacy and toxicity was then observed. Results: 1?One the eighth day of the first treatment cycle, the patient appeared dyspnoea(CTC class II), scattered skin rashes in the trunk and extremities(CTC class II),.Type-B ultrasonic examination for chest indicated that there were large quantity pleural effusion in right,which was demonstrated exudate by pleural puncture drainage.The CRP content was 180ug/L in blood. the rash and effusion subsided after a week. 2?After 2 cycles of treatment, we found that primary lung tumor shrank 31.1% and the brain lesions disappeared.The overall evaluation was PR. 3?4 months after 4 treating cycles, We found aplastic anemia through a bone marrow biopsy,because the patient occurred severe anemia; imaging examination found that the lung site tumor advanced and a new lesion appeared in the Left adrenal gland. 4?Left adrenal lesion puncture biopsy suggested that tumor cells were surrounded by NK cells. Conclusions:There was no significant toxicity during the treatment of Pemetrexed combining DC-CIK for the patients with advanced EGFR wild-type Lung adenocarcinoma in vivo.We found the phenomenon that new tumor site was surrounded by NK cells.And it laid the foundation for the next step for the large sample clinical research.
