Effect Of Psychological Intervention On Depression Anxiety And Quality Life Of Gastrointestinal Cancer Patients Undergoing Chemotherapy

Objectives: To understand the cognition degree, acceptability, worry and psychological states of gastrointestinal cancer patients on chemotherapy, to measure the depression, anxiety and quality of life of research subjects, to explore the effects of individualized psychological intervention on depression, anxiety and quality of life of gastrointestinal cancer patients undergoing chemotherapy, to evaluate the effectiveness of psychological intervention.Methods:In this study, a large number of related literature were carefully read and reviewed, 100 gastrointestinal cancer patients undergoing chemotherapy as research subjects, which were collected from the general surgery and oncology of a top three hospital in Qiqihar city, were randomly assigned into psychological intervention group(general surgery ward, 50 cases) and control group(oncology ward, 50 cases), and the chemotherapy cycle of all patients was 6 months(The study adopted FOLFOX4 chemotherapy project, once after two weeks and totally 12 times). Using questionnaires and interviews to understand the economic conditions, the cognition degree, acceptability, worry and psychological states of the two groups of patients on chemotherapy, and develop individualized psychological intervention measures in the intervention group patients according to their specific circumstances. The content of intervention mainly includes psychological intervention based on daily supportive psychological care. The psychological interventions such as health education,cognitive behavioral therapy,collective psychological intervention,support therapy,social support in the form of intervention.in early days, mid-term(before 7th chemotherapy) and later stage, respectively..It evaluated the depression, anxiety and quality of life of the two groups of research subjects in Pre-, Mid- and Pro-intervention by questionnaires including Self-rating depression scale, Self-rating anxiety scale and SF-36, thus compared and analyzed the effectiveness of psychological intervention.Results:(1) In view of object cognitive level: Awareness of chemotherapy in observation group was 72.0% before intervention, and the figure in acceptance and worried about side effects of chemotherapy were 62.0% and 46.0% respectively. By contrast, the figure in control group were 73.5%, 59.2%, 38.8% correspondingly, there was no statistically significant difference(p>0.05). The rate of recognition, acceptance and worried about side effects in observation group were 76.0%, 76.0% and 32.0% respectively after psychological intervention. In comparison, the figure were 28.6%, 34.7% and 79.6% correspondingly. Intervention in two groups of patients with late awareness, acceptance, and concerns about side effects are improved, the difference was statistically significant(P<0.05). There were statistical significance of the two groups among the three period of intervention(P<0.05).(2) In view of anxiety, depression level: According to the results of this study of 100 patients with malignant tumor of the depression, anxiety questionnaire score was significantly higher than normal; incidence of depression, anxiety and other bad mood are 71.4%, 59.2% and 70.0%,62.0% in the two groups before intervention, there was no statistically significant difference(p > 0.05), SDS score in the control group was 49.01 ±5.08, SAS score was 44.73± 3.90, and 50.32± 4.76, 42.03 ± 4.11 in observation group respectively, there was no statistically significant difference(p>0.05). Changing mood in observation group had improved compared with before intervention, statistically significant difference(p<0.05), while in control group of chemotherapy the depression, anxiety increased(p<0.05).(3) From life quality point of view: The physiological function of observation group before intervention was(42.2±15.5), physiological function(48.5±15.3), body pain(47.2±13.7), and overall health(54.9±10.1), social function(54.5±10.1), there is no statistical differences(p>0.05) compared with control group, mid intervention?late intervention group except mental health,in physiological function, physiological function, body pain, energy, social function, and other aspects of the social function score significantly higher than that of before intervention, it was statistically significant(p<0.05), the late intervention group than the control group's quality of life status has improved significantly(p>0.05).Conclusions :(1)Gastrointestinal malignant tumor chemotherapy patient awareness to generally is not high, some patients such as without psychological intervention in the treatment of mid may appear cognitive deviation, lead to counterproductive to treatment and worried about side effects. After the effective psychological intervention, can be gradually improved gastrointestinal malignant tumor chemotherapy in treating patients with recognition and acceptance, to reduce side effects.(2) The status of various functions of gastrointestinal cancer patients, including physical function, emotional function, role function and overall life, have been improved to some extent after individual psychological interventions,make its better treatment compliance, maintain good states of mind during the treatment.(3) Individualized psychological intervention can improve emotional status of gastrointestinal cancer patients and make them take a positive attitude in the course of chemotherapy, and it plays a significant synergistic effect on the treatment and it is of actual clinical significance to improve the quality of life of patients.
