ObjectivesTo investigate the subthreshold depression and the quality of life in patients preliminarily diagnosed with breast cancer,and to explore their relationship.Also discuss the effects of group psychological intervention on subthreshold depression and quality of life in newly diagnosed breast cancer patients,and construct the professional,systematic group psychological interventions for breast cancer patients in this region to improve their quality of life.MethodsThe study is divided into two parts: The first part is a cross-sectional study to investigate the subthreshold depression and quality of life of newly diagnosed breast cancer patients.The second part is the implementation and effect evaluation of group psychological interventions.By adopting a convenient sampling method,a newly diagnosed breast cancer patient treated in the breast department from January to December 2017 in one Three Level of First-class hospital in Jingzhou was selected as the research object,and a self-designed general data questionnaire was used.Centre for Epidemiological Studies Depression Scale(CES-D),which is currently used at home and abroad,and the most prevalent assessment of depression in the Hamilton Scale of Depression Scale(Hamilton Rating Scale for Depression,HAMD-17): This form is only used for screening for subthreshold depression for CES-D scores >16 points.The Clinical Assessment of Cancer Therapy-Breast(FACT-B)is used to determine the quality of life of patients with subthreshold depression.According to the established group psychological counseling program,53 newly diagnosed breast cancer patients diagnosed as subthreshold depression between January and June 2017 were divided into the control group according to the patient’s admission time,and the routine psychological care and health were performed.49 newly diagnosed breast cancer patients who were diagnosed with subthreshold depression between July and December 2017 were divided into intervention groups.Group psychological intervention was performed every 2 times a month for 60 to 90 minutes for a total of six times month.The CES-D scale and FACT-B scale were used to evaluate the changes of subthreshold depression and quality of life indicators in the two groups before and after the intervention.Statistical analysis was conducted to determine the significant impact on group psychological intervention for the quality of life of newly diagnosed breast cancer patients with subliminal depression.Results 1.The incidence of subthreshold depression in newly diagnosed breast cancer patients was 48.6%.The subthreshold depression of newly diagnosed breast cancer patients were significantly correlated with different ages,marital status,family monthly income,working status,educational level,basic diseases and treatment methods between patients with and without depression(p<0.01),but there were no significant differences in religious beliefs and medical payment methods(p>0.05).2.The average quality of life score of newly diagnosed breast cancer patients with subthreshold depression was(73.92 ±17.62).The scores of non-depression were higher than those of subthreshold depression(p<0.01).3.The CES-D score of newly diagnosed breast cancer patients was negatively correlated with scores of the total and various fields of quality of life(p<0.01).The quality of life of patients could be affected by subliminal depression to a certain extent.The areas affected by subliminal depression were social/family status,additional status,and less affective status.4.There was no significant difference in CES-D score between group psychological intervention and control group before intervention(p>0.05).The study on group psychological intervention of subliminal depression showed that scores of CES-D were significantly lower than that before intervention(p<0.05).There was no significant difference in CES-D scores of control groups between before and after intervention(p >0.05).5.There was a statistical difference in the quality of life dimensions and total scores between the intervention and the control group after group psychological intervention(p<0.05).Conclusions 1.The incidence of subthreshold depression in newly diagnosed breast cancer is high.Young and middle-aged,low-income,non-married,non-employed,basic diseases and low education are risk factors for the occurrence of subthreshold depression in newly diagnosed breast cancer patients.Subliminal depression can affect the quality of life of patients,we should pay attention to the subliminal depression of patients at this stage,and pay more attention to the psychological problems of patients in an early and comprehensive way.2.Group psychological intervention can effectively reduce the incidence of depreesion,improve the quality of life in newly diagnosed breast cancer patients,it can be used in the clinical to help patients cope with their early psychological problems effectively.
