Observational Study Of The Macular Area Of Patients With High Myopia After The Cataract Surgery By Optical Coherence Tomography

Objective: To study the correlation between macular foveal retinal thickness(FRT)and the best corrected visual acuity(BCVA)in high myopia patients with various axial length,whounderwent phacoemulsification(PHACO)combined foldable intraocular lens implantation before surgery,1 week,1 month and 3 months after surgery,respectively,by optical coherence tomography(OCT).Methods:Total 63 patients(80 eyes)were included in this study,who underwent PHACO combined foldable intraocular lens implantation in the First People's Hospital,Taicang City,from January 2016 to May 2016.Among these patients,32(39 eyes)were male and 31(41 eyes)were female,with the age ranging from 48 to 79 years old(the average 63.52±7.56 years old).Bestides,32 patients(48 eyes)had the high myopia with the axial length >26mm,and 31(32 eyes)were the senile cataract with the axial length ranging between 22 mm and 26 mm.Based on the axial length,the patients were divided into 3 groups as follows: Group A(22mm<axial length<26mm),with 16 male(16 eyes)and 15 female(16 eyes);Group B(26mm<axial length ?28mm),with 10 male(14 eyes)and 11 female(16 eyes);and Group C(axial length>28mm),with 6 male(9 eyes)and 5 female(9 eyes).There were no significant differences among 3 groups in terms of age and sex(P>0.05).The FRT as well as BCVAof the operated eyes were recorded before and after surgery(1 week,1 month and 3 months postoperatively)by OCT and international visual acuity,respectively.The correlation between FRT and BCVA was analyzed using SPSS 19.0 statistical software.A P-value of 0.05 or less was considered statistically significant.Results:1.The FRT in each group before PHACO was as follows: Group A,226.00±10.00?m;Group B,208.75±6.65?m;andGroup C,199.00±14.14?m,respectively.Compared with that in Group A,the FRT in Group B and C was significantly decreased(P<0.05).2.Compared with that before surgery,the BCVA in 3 groups were greatly increased 1 week,1 month and 3 months after PHACO.The BCVA in Group A is significantly higher than that in Group B and C(P<0.05).3.There was significant difference in BCVA detected at 1 week,1 month and 3 months(P<0.05);BCVA>0.3 accounted for 96.88%,83.33% and 44.44%,respectively,in 3 groups;and the axial length was negatively correlated with postoperative BCVA.The difference in BCVA among all groups after surgery showed no statistical significance(P>0.05).4.There was no difference in FRT inGroup A before and after surgery(P>0.05).However,the FRT in Group B and C was significantly thickened 1 month after surgery compared with that before surgery(P<0.05);and were reduced 3 months after surgery compared to that 1 month after surgery(P<0.05).Conclusion :1.Compared with that with simple senile cataract patients,the FRT was much thinner in patients with high myopia(axial length>26mm)detected by OCT.2.BCVA in 3 groups wassignificantly improved after surgery compared with that before surgery,indicating a safe and effective procedure with PHACO combined foldable intraocular lens implantation.3.The axial length examined by A-ultrasound and FRT by OCT before cataract surgery can partly predict the outcome of the postoperative visual acuity of patients.4.Patients with high myopia(axial length>26mm)are likely to develop macular edema,thus increase FRT 1 month after PHACO surgery,whichwas reduced 3 months after surgery.These resultsindicated the internal environment of the eye gradually becomes stable with time,the macular edema is alleviated,and some patients improved BCVA after surgery.The increase of postoperative FRT is negatively associated with postoperative BCVA.Thus,OCT examination is very useful to provide reliableinformation for patients with high myopia,who underwent cataract surgery.
