Analysis Of Related Factors On Influencing The Healing Of Diabetic Foot Ulcer And Amputation Of Foot Ulcer

Objec t ive :To analysis the influencing factors of healing and amputation of dia betic foot ulcers.Met hods :We collected 308 cases of patients with diabetic foot ulcer in a tertiar y hospital of Anhui province from September 2005 to September 2015,who we re admitted(male 177 cases,female 131 cases,age 23~91 years old(63.02 + 12.89),duration of diabetes was 0 ~ 30(10.5± 6.98)years;Type 1 diabetes mell-it us patients with 11 diabetes,Type 2 diabetes mellitus patients 297 cases of urina ry disease)clinical data and laboratory test results were retrospectively analyzed.According to whether the foot ulcer healed or not,it was divided into the heal ing group and the non healing grou p.In accordance with the foot ulcer is divid ed into amputation group and no amputation group.Res ults :(1)The dependent variable for healing,by single factor Logstic regress ion analysis showed that the healing group age,Wagner grade,VLDL,maximum area of ulcer,hypertension,diabetic nephro pathy,and cerebrovascular disease we re higher than those of non healing grou p(OR=1.003,0.996,1.006,3.445,P<0.05),while Hb,PA,education level,e GFR,were lower than that of the non heali ng group(OR=1.108,1.034,3.193,1.049,P<0.05),The beat of the dorsal artery of the foot was worse than that of the non-healing group(OR=36.078,P<0.05),non-healing group have effective debridement treatment less than healing group(OR=2.998,P<0.05).(2)Toamp utation as the dependent variable,The single facto r Logstic regression analysis showed that The amputation group of platelets,UA, WBC,Wagner classification were higher than the non amputation group(OR=1.003,0.996,1.006,3.445,all P<0.05),The proportion of recurrent ulcers in the li mb group was higher than that in the non amputation group(OR=2.275,P<0.05);Whereas the amputation gro up of apo A,HDL was lower than that of the non a mputation group(OR=0.341,0.213,both P<0.05);Whereas the amputation group of apo A,HDL was lower than that of the non amputation group(OR=0.341,0.213,both P<0.05);The ratio of Neuropathic foot ulcer in the amputation group was l ower than that in the non amputation group(OR=0.363,P<0.05),The amputati on group was treated with negative pressure suction,which was lower than that of the non amputation group(OR=5.185,P<0.05).(3)the protective factors of fo ot ulcer healing include: hemoglobin,pre albumin,glomerular filtration rate(e GF R),educational level,the beating of the dorsal foot,debridement.The risk factor s of foot ulcer healing were: hyperten sion(HBP),age,Wagner classification,ver y low density lipoprotein(VLDL),the largest area of ulcer,the stage of diabetic nephropathy.Among them,the pre albumin,debrid ement,and foot dorsal artery pulsation were the protective factors for the healing of foot ulcer,and very low density lipoprotein(VLDL)and diabetic nephropathy stage were the independent risk factors for diabetic foot ulcer healing.conc lus ion To improve the nutritional status of patients,to pay attention to the pulse of th e dorsal foot artery,control blood pressure,blood lipids,and timely debridement for diabetic foot ulcer healing is very important.Positive effective negative pres sure suction therapy,control of diabetic foot ulcer inflammation for the disease t o avoid further deterioration of the disease,reduce the diabetic foot amputation has a crucial role.
