The Utilization Of Maternal Health Services Among Maternal Women In Guangde County,Anhui Province

Objective: To identify the utilization of maternal health services among maternal women in Guangde county,and the factors of service utilization,to identify the consistence between maternal records and questionnaire survey,and provide reference for further improvement among maternal health services.Methods: By using stratified cluster sampling method,one village/street was randomly selected from each of the 10 townships/communities.All maternal women delivering at the time from Oct.1,2012 to Sept.30,2013 were interviewed with structure questionnaire.All maternal health records were also collected.Results: Among the 350 maternal women,the rate of prenatal care at early pregnancy was 82.3%,the rate of filing records in first trimester was 46.0%,the coverage of prenatal care(5 or more times)was 96.6%,the coverage of antenatal care was 100%,the rate of hospital delivery was 100%,the rate of postpartum visit was 76.3%,and the rate of systematic maternal health management was 64.9%.Among the 288 women receivied early prenatal care,79.9% were received blood testing,testing of HIV,syphilis and hepatitis B,testing of liver function.The main reasons for not utilizing early prenatal care and not filing records were unaware of prenatal care(44.2% and 38.8%)and living out-of-town(29.1% and 27.0%).The rates of both early prenatal care and filing records among women detected pregnancy at local maternal and child health(MCH)center were significantly higher than those detected pregnancy at medical centers.The rates of both early prenatal care and filing records were both associated with family planning and living place at early pregnancy(p<0.05).The coverage of prenatal care(5 or more times)was associated with education,average family income and family planning(p<0.05).The rate of postpartum visit was associated with education and delivery place(p<0.05).The rate of systematic maternal health management was associated with education,living place at early pregnancy,occupation at early pregnancy,place of prenatal care,and delivery place(p<0.05).The rate of full compliance between the content of maternal health services manual records and the questionnaire results was 59.3%,part of the rate was 40.7%.The consistency rates of filing records at early pregnancy,early prenatal care,number of prenatal care,and number of postpartum visits were 97.0%,85.1%,90.3% and 79.1%respectively.Full compliance rates were associated with the maternal age,the place of first trimester pregnancy,the primary site of antenatal care,and the place of delivery.Conclusion: Maternal health services among maternal women need to be further improved in Guangde County.The rates of both early prenatal care and filing records were both associated with family planning and living place at early pregnancy.The coverage of prenatal care(5 or more times)was associated with education,average family income and family planning.The rate of postpartum visit was associated with education and the delivery place.The rate of systematic maternal health management was associated with education,living place at early pregnancy,occupation at early pregnancy,place of prenatal care,and delivery place.The rate of consistence between maternal records and questionnaire survey was low.Which was associated with living maternal age,places at early pregnancy,places of prenatal care,places of delivery.It is suggested that effective measures such as publicity,management and training should be taken to improve the service quality of health care institutions in Guangde County and the awareness of maternal health care services so as to improve the rate of maternal health care services in Guangde County.
