Objective:To observe the effect of vitrectomy on corneal epithelium in diabetic retinopathy patients,and to investigate the effect of corneal bandage lens on corneal epithelial repair.Methods:From January 2014 to January 2016 in our hospital diagnosis and treatment of 46 patients who underwent the vitreous body after resection of persistent corneal epithelial defect of diabetic retinopathy patients as the research object.The patients were divided into the control group and the experimental group,each with 23 cases.The control group was treated with routine drug treatment,patients in the experimental group on the basis of routine therapy combined with the use of bandage mirror treatment.observe the symptom of comfort,pain and corneal epithelial healing after treatment in two groups to evaluate the clinical effect of bandage assisted by conventional drug therapy.Results:During follow-up,there were 46 cases(46 eyes)of eligible patients were observe,observation group and control group,each 23 cases(23 eyes).Preoperative observation group average BUT was 7.81± 3.21s,the control group was 8.01± 2.10s,the difference between the two groups was not statistically significant(t=0.234,P=0.612);the observation group Schirmer test average was 10.73±6.12mm,11.02± 5.62mm control group,the difference between the two groups had no significant difference(t=0.306,P=0.412).The corneal epithelial healing time of the control group of patients was 5 days to 62 days,21.6±11.2 days on average,including 11 cases of the persistent corneal epithelial defect,the observation group was 3 days to 42 days,13.3± 8.4 days on average,including 5 cases of persistent corneal epithelial defects over 2 weeks.The difference between the two groups was statistically significant(t=2.631,p=0.003).Two groups of patients after treatment,the number of healing cases in first week(11 cases in the observation group,accounting for 47%,4 cases in the control group,accounting for 17.3%),In the second week(18 cases in the observation group,accounted for 78.26%,12 cases in the control group,accounting for 52.1%),The corneal epithelial healing time of the observation group was shortened,but the epithelial healing time of 1 case in the observation group is more than 6 weeks.Compare the ocular comfort rating in the first week.Patients with ocular foreign body sensation:observation group 0.34±0.27,0.53±0.22 in control group(?2=2.6163,P=0.0121);pain:observation group 0.69±0.25,0.96±0.21 in control group(?2=3.966,P=0.0003);photophobia:observation group 0.41±0.39,0.65±0.33(control group?2=2.253.P=0.0293),the observation group was significantly better than the control group.With the gradual healing of corneal epithelium,the comfort rating in the late gradually is close,but the first 3 weeks of the score is still statistically significant difference.Comparison of eye pain.In the first week the observation group have 22 cases(95.65%)in grade 0.5,only 1 case in grade 1,0 case in grade 2,and the control group have 16 cases(69.5%)in grade 0.5,6 cases in grate 1(26.09%),1 case in grade 2(4.35%).The observation group in grade 0.5-1 of pain is obviously superior to the control group(0.5?2=4.072,1 P=0.044;?2=16.133,P=0.000).Conclusion:Persistent corneal epithelial healing is common ocular complications of patients with diabetic retinopathy after vitrectomy.Bandage contact lens can significantly promote the corneal epithelial healing of patients after vitrectomy for diabetic retinopathy,and plays an important role in reducing eye symptoms and improving the visual quality of patients.
