Herpes simplex keratitis is one of the most common infectious keratopathy.In recent years,the incidence of the disease has been significantly increased,and it has risen in the first place in the blinding keratopathy.The conventional treatment is the drug treatment: the use of antibiotics,hormones and mydriatic eye drop.When drug treatment does not control the course of the disease,surgical treatment is required: amniotic membrane transplantation / corneal transplantation.A new means of adjuvant therapy is present on the market: a corneal bandage lens.It is a kind of lens made of special materials,soft,simulating the anterior corneal surface,directly attached to the tear layer of the corneal surface and can be compatible with the human eye,so as to achieve the purpose of treatment.The aim of this study is to evaluate the safety and efficacy of BSCL in the treatment of refractory corneal ulcer caused by epithelial HSK,in order to select a more reasonable treatment for these patients.Objective:To evaluate the safety and efficacy of corneal bandage soft contact lens(BSCL)in the treatment of refractory corneal ulcer caused by herpes simplex keratitis(HSK).Methods:22 patients(22 eyes)with refractory corneal ulcer caused by HSK were randomly divided into 2 groups: group A were covered with silicon hydrogel BSCL and group B were served as control,who were interviewed before treatment and on 1,2 and 3 week after treatment.Their ocular surface disease index(OSDI)questionnaires and visual analogue scores(VAS)were scored,and uncorrected visual acuity,corneal fluorescein staining and anterior segment optical coherence tomography(AS-OCT)were taken.Results:The final visual acuity in group A was significantly higher than that of the group B after treatment.The OSDI scores decreased rapidly with time in two groups,which were significantly lower in group B than that in group A(P<0.05).The VAS scores at each time point after treatment in group A was lower than that in group B(P<0.05).The area and depth of ulcer at each time point in group A were significantly lower than that in group B(P<0.05).The area of neovascularization increased in group A on week 1 after treatment,and significantly decreased thereafter,while there was no differences at each time point in group B(P>0.05).The aera of neovascularization in group A was significantly higher than that of group B on week 1,which were significantly lower than that of group B on week 2 and week 3(P<0.05).There was no adverse reaction during the follow-up,except for one case who lost the lens.Conclusion:Corneal bandage soft contact lens is safe and effective in the treatment of refractory corneal ulcers caused by HSK.
