Objective:The purpose of this study is to investigate the self management status, medication compliance and medical status of patients with coronary heart disease after percutaneous coronary stenting, and to identify and explore the main factors influencing the status quo.Research methods:This study is a descriptive study.180 patients with coronary stent implantation in a3A hospital in Zhejiang Province from June 2014 to March 2015were performed as objects of study. The questionnaire of self-management of coronary heart disease, medical coping and medication compliance questionnaire were used to investigate. To analyze the main influencing factors of self management of patients after coronary stent implantation.Spss20.0 statistical software, descriptive statistics analysis, single factor variance analysis,T-4est and multiple linear regression analysis method were applied to analyzethe data, measurement data(mean ± standard deviation)shows that two or more groups of mean by chi square test or Fisher’s exact test, medication compliance score appears a skewed distribution, and using the non parametric Mann Whitney U test and Kruskal Wallis test(P<0.05)were considered statistically significant.Research results:1.A total of 180 cases of patients with PCI in a 3A hospital in Zhejiang province were investigated,180 questionnaires were issued,176 questionnaires were recovered,10 questionnaires were excluded,166 valid questionnaires were returned, and the effective questionnaire recovery rate was 92.2%.2.1 The average score of self-management of patients after coronary stent implantation is 69.50±10.98,rating for the general. The score of life behavior management is 87.36±9.71,rating for fairly good. The score of emotional management is 77.71±17.66,rating for fairly good. The score of disease medical management is 57.51±14.95,rating for the general.2.2The analysis of single factor variance and lologistic analysis shows that different occupations, different educational levels and gender differences create significant differences in the total score of self-management among patients(P<0.05).The score of patients in administrative institutions higher than other professional, the statistical difference issignificant(P<0.01);The higher education degree, the higher score is,the statistical difference issignificant(P<0.01);The self-management score in female patients is higher than that of male patients, the statistical difference is significant(P<0.05).Emotional management:Patients in Administrative institutions score higher, and the difference is statistically significant(P=0.006).The score of the female patient is higher than the male patient, the statistical difference issignificant(P=0.005).Disease management:Patients in administrative institutions score higher than farmer patients, the statistical difference issignificant(P<0.01).Patients with college degree have a higher score, the higher education degree, the higher score is,the difference is statistically significant(P<0.01).3.Medication compliance score of patients after coronary stent implantation is 7.51±1.35,at a moderate level.According to Logistic regression analysis, there is of statistic significant in the different scores of medication compliance among patients with different levels of education, stent implantation(P<0.01).Medication compliance of patients with primary and secondary education are better than the patients with college degree;Medication compliance of patients with the first PCI more than 3 years is better than that of those within one or two years. The medication compliance score was positively correlated with the scores of life behavior management and disease medical management(P<0.05).4.Coping style of patients after coronary stent implantation, facing accounts for 39.8%,38.6%of yielding,21.6%of avoiding. The use of chi square test or Fisher exact test shows that there are significant differences in the choice of medical coping style among patients with different occupation, education level, living location, disease management level and emotional management level(P<0.05).Patient characteristics:High degree of education,administrative/business unit, living place in the city, aged 40 to 49 patients choose to face high proportion,Aged 30-39 patients choose to avoid high proportion; The proportion of women choose to yield high,The difference was statistically significant(P<0.05).The choice facing accounts is positively correlated with emotional management and disease medical management.Research conclusion:1.Patients with coronary stent implantation do have problems in self-management.The score of self-management of patients with coronary stent implantation rating forgeneral,it will get better still, the characteristics of different patients affect the results of self-management score, medical staff needs to pay attention to influencing factors of self-management of patients with coronary stent implantation, and Improve patient self-management rating.2.Medication compliance score of patients after coronary stent implantation is at a moderate level. The characteristics of different patients affect the results of medication compliance score, medication compliance score was positively correlated with the scores of life behavior management and disease medical management.Medical staff needs to pay attention to the influencing factors of medication compliance, and Improve patient self-management rating. To reduce the rate of restenosis after coronary stent implantation.3.The proportion of positive coping style(face)is low in patients with coronary stent implantation. The choice facing accounts is positively correlated with emotional management and disease medical management. The characteristics of different patients affect the medical coping.Medical staff needs to pay attention to the influencing factors of medical coping, and improve patient self-management rating and the quality of life of patients after stent implantation.
