| Objective1 To understand the current status of self-perception burden,coping style and self-management ability of patients with percutaneous coronary stent implantation.2 To analyze the correlation between the self-perception burden,coping style and self-management ability of patients with percutaneous coronary stent implantation.3 To explore the mediating effect of coping style on the self-perception burden and self-management ability of patients with percutaneous coronary stent implantation.4 To provide reference for the implementation of effective interventions to alleviate the self-perception burden of patients with percutaneous coronary stent implantation,change coping styles,and improve self-management ability.MethodA cross-sectional survey was conducted to select 280 patients who underwent percutaneous coronary stenting in a third-grade hospital in Dalian from November 2017 to November 2018.The patients were investigated when they were discharged from the hospital.Research tools include general data questionnaires,the self-perception burden scales,medical coping style questionnaires,and self-management capacity scales for coronary stent implantation.After data collection,SPSS22.0 and Amos23.0 were used for descriptive analysis,Pearson correlation analysis,multiple stepwise regression analysis,and structural equation model analysis.The difference was statisticallysignificant at P<0.05.Result1 The self-perceived burden score of patients with percutaneous coronary stent implantation was(25.93±8.17)points.The scores from low to high were economic(2.47±1.43)points,emotion(2.55±0.97)points,body(2.68 ±0.95)points.2 Patients with percutaneous coronary stent implantation scored of the Avoidance and,resignation and Confrontation were(2.15±4.36),(1.92±0.61),(2.22±0.84).The patient’s Confrontation score was higher than the domestic norm,and the avoidance and resignation scores were lower than the domestic norm.The difference was statistically significant(P<0.05).3 The self-management ability score of patients with percutaneous coronary stent implantation was(55.95±7.64)points.The scores from low to high were disease knowledge(1.35±0.60)points,first aid skills(2.01±0.73)points,daily.Life knowledge(2.52±0.35)points,emotional management(2.56±0.43)points,according to medical behavior(2.71 ± 0.55)points.4 Age,smoking,payment method,family per capita income,Place of residence,number of stent implantations,first stent implantation time,number of stents,and knowledge of risk factors.Differences in self-management ability of patients with percutaneous coronary stent implantation Statistically significant(P<0.05).5 There was a negative correlation between self-management ability and self-perception burden in patients with percutaneous coronary stent implantation(r=-0.454,P < 0.001);self-management ability was positively correlated with Confrontation(r =0.391,P< 0.001);The avoidance was negatively correlated(r =-0.433,P<0.001);negatively correlated with resignation(r=-0.322,P<0.001).The self-perception burden was negatively correlated with the Confrontation(r=-0.228,P< 0.001);positively correlated with avoidance(r = 0.412,P < 0.001);positively correlated with resignation(r=0.297,P<0.001).6 Risk factors,stent placement,age,place of residence,self-perceived burden,avoidance,resignation,Confrontation regression model in the self-management abilityof patients with percutaneous coronary stent implantation can explain 48.2% of the total variation in self-management ability.7 Structural equation model analysis showed that coping style mediates the self-perceived burden and self-management ability of patients with percutaneous coronary stent implantation.Self-perception burden can directly affect self-management ability,and can also indirectly affect self-management ability through avoidance,resignation,and confrontation.Conclusions1 The self-perceived burden of patients with percutaneous coronary stent implantation is at a mild burden level.2 Most patients with percutaneous coronary stent implantation adopt confrontation coping style,followed by avoidable coping style,and less use of resignation coping style.Medical staff guide patients with avoidable coping styles,which will improve the overall coping level of patients with percutaneous coronary stent implantation.3 The status of self-management ability of patients with percutaneous coronary stent implantation is at the middle-lower level.The score of disease knowledge dimension is the lowest,and the medical staff should pay attention to it;the highest score of compliance behavior,but there is still room for improvement.4 The self-perceived burden,avoidance and resignation coping styles of patients with percutaneous coronary stent implantation were negatively correlated with self-management ability;confrontation coping style was positively correlated with self-management ability;and self-perceived burden,avoidance and resignation response The method can predict the self-management ability negatively,and the confrontation coping style can positively predict the self-management ability.5 Knowing the risk factors,the number of stent implantations,age,place of residence,self-perceived burden and coping style are the main influencing factors of self-management ability of percutaneous coronary stent implantation.6 Coping style mediates the self-perceived burden and self-management ability of patients with percutaneous coronary stent implantation.Self-perception burden candirectly affect self-management ability,and can also indirectly affect self-management ability through avoidance,confrontation and resignation. |
