Research On The Influences Of Abolishing Drug Price Markups Policy On Provincial Public Hospital Operation And Countermeasure Analysis

As life changes,there is an increasing demand for medical treatments and health services.High costs and difficulty in getting access to medical services are highlighted,and more and more attentions are paid to the supply side reform of the medical service system.The policy of abolishing drug price markups,part of the reforms in public hospital,is the core of the supply side reform of the medical service system.In this study,the influences of abolishing drug price markups policy on provincial public hospital operation and countermeasure analysis are taken as the object of study.The case study of RH comprehensive Third-level Grade A hospital in Shandong province is carried out.The study gives comprehensive introduction to the origin of the drug price markups policy,the adverse effects of the drug price markups policy on medical service system,the process of the abolishing drug price markups policy in China,especially Shandong province,and its influence on the various stakeholders.The HIS report data from RH,a comprehensive Third-level Grade A hospital in Shandong province have been collected ranging from June to November 2016 and from June to November 2015.Trend analysis was adopted to compare differences in turnover,revenue,revenue structure and average expenses in the same period before and after abolishing drug price markups in order to reflect the influence of the policy on provincial public comprehensive hospitals.The success experience and the deficiencies in the process of abolishing drug price markups in RH hospital were cited as a reference to related management and follow-up policy enactment.After abolishing drug price markups policy was set in motion,the outpatient and emergency visits of RH Hospital decreased by 7.36%compared to the same time of last year,while the number of discharged patients increased by 9.44%and that of operations increased by 13.17%.The medical income increased by 2.64%compared to that of the same time of the last year.Medicine proportion decreased from 44.04%to 37.35%;however,the net medical income proportion increased by 4.20%,but sanitary materials income increased by 12.66%;sanitary materials income proportion increased by 2.21%.The average outpatient expenses increased by 7.04%,while the average hospitalization expenses reduced by 5.11%.The compensation rate of price adjustment was 87.05%.As a comprehensive Third-level Grade A Hospital,it had leading medical technology and great admissions capacity,the turnover was rarely affected by a certain range of medical services price adjustments.The abolishing drug price markups policy had little effect on the total medical revenue of RH hospital,which was conductive to the optimization of income structure,promotion of service value of the medical staff and their enthusiasm,efficiency of hospital operation and control of drug use and hospitalization expenses.Meanwhile,some problems emerged in the implementation of the policy in RH hospital.There was a shift in different kinds of costs,which made the average outpatient expenses keep increasing while patients had weak sense of recognition.The price reform of medical services was still not systematic.The sanitary materials expenses grew and the pharmacy department made little revenue.The hospital needs to further strengthen the internal management and cost control.Based on the performances of the implementation of the policy on RH hospital,the paper suggests putting in place government fiscal responsibility,extensive promotion of "two votes",supply side reform of pharmaceutical industry,gradually improvement of the medical service price system,the establishment of a more balanced compensation mechanism under the guidance of "Three medical linkage",the strengthened control of sanitary materials use,the fair treatment of the pharmacy work and sustainable development of public hospitals with strengthened internal management going forward in the public hospital comprehensive reform to reduce the medical burden on the masses.Since the urban public hospital reform in Shandong province takes the lead in China,this study investigates the first hand data of comprehensive reform in provincial public hospital in Shandong province to serve as a reference for policy enactment and management of reforms in subsequent provincial public hospitals.
